研究目的:

精准构建并验证急性A型主动脉夹层术后机械通气患者撤机风险的预测模型,切实提高撤机成功率。通过引入先进模型并深入剖析风险因素,精准评估患者术后风险,优化术后风险管理体系,缩短呼吸机使用时长及住院周期,为ATAAD患者术后康复进程加速,有效缩短住院周期,降低医疗成本及患者身心痛苦,减少医疗纠纷,实现经济效益与社会效益的双赢。  

Objectives of Study:

Accurately constructing and validating a risk prediction model for weaning from mechanical ventilation in patients after acute type A aortic dissection surgery to effectively improve the success rate of weaning. By introducing advanced models and thoroughly analyzing risk factors, the model enables precise assessment of postoperative risks, optimizes the postoperative risk management system, shortens the duration of mechanical ventilation and hospital stay, accelerates postoperative recovery in ATAAD patients, effectively reduces hospitalization time, lowers medical costs and physical and psychological burdens, decreases medical disputes, and achieves a win-win outcome in terms of both economic benefits and social value.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.接受急性A型主动脉夹层术后机械通气的患者; 2.年龄≥18岁; 3.患者撤机前评估神志清醒,肌力可,血流动力学指标平稳,经治 疗组医师评估具备撤机条件; 4.撤机前均行自主呼吸试验 (spontaneousbreathingtrial,SBT)筛查; 5.患者或家属知晓并签署知情同意书;

Inclusion criteria

1.Patients receiving mechanical ventilation after acute type A aortic dissection surgery; 2.Aged >=18 years; 3.Assessed as awake, with adequate muscle strength and stable hemodynamic parameters prior to weaning, and deemed suitable for weaning by the attending physician; 4.Undergoing a spontaneous breathing trial (SBT) screening before weaning; 5.Patients or their family members have been informed and have signed the informed consent form.

排除标准:

1.血流动力学极不稳定,需要大剂量血管活性药物维持生命体征者;
2.严重感染、脏器功能障碍者;
3.既往有脑血管及神经系统疾病 病史者;
4.膈肌损伤及瘫痪者;
5.中途退出本研究。

Exclusion criteria:

1.Patients with extremely unstable hemodynamics requiring high-dose vasoactive agents to maintain vital signs;
2.Patients with severe infection or organ dysfunction;
3.Patients with a history of cerebrovascular or neurological diseases;
4.Patients with diaphragmatic injury or paralysis;
5.Patients who withdraw from the study prematurely;
