研究目的:

针对我国儿童急性下呼吸道感染病原学监测及重症肺炎预警体系尚不完善,且8岁以上肺炎支原体肺炎患儿精准治疗存在困境的现状,本研究旨在构建儿童急性下呼吸道感染病原学动态监测系统、开发儿童重症肺炎智能预警体系,并开展8岁以上肺炎支原体肺炎患儿多西环素治疗的临床研究,旨在为提升儿童急性下呼吸道感染的精准防控、早期预警及优化治疗提供系统性解决方案与高级别循证依据。  

Objectives of Study:

Given the current situation in China where the pathogen monitoring for acute lower respiratory tract infections in children is not yet complete, and there are difficulties in providing precise treatment for children with severe pneumococcal pneumonia, this study aims to establish a dynamic monitoring system for the pathogens of acute lower respiratory tract infections in children, develop an intelligent early warning system for severe pneumonia in children, and conduct a clinical research on doxycycline treatment for children with pneumococcal pneumonia over 8 years old. The goal is to provide systematic solutions and high-level evidence-based support for improving the precise prevention and control, early warning, and optimized treatment of acute lower respiratory tract infections in children.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.根据《儿童肺炎支原体肺炎诊疗指南(2025年版)》及《儿童肺炎支原体肺炎诊治的专家释疑》诊断为肺炎支原体肺炎的患儿; 2.年龄>=8岁; 3.病程<=5天

Inclusion criteria

1. Children diagnosed with mycoplasma pneumonia according to the "Diagnosis and Treatment Guidelines for Mycoplasma Pneumonia in Children (2025 Edition)" and the "Expert Clarification on Diagnosis and Treatment of Mycoplasma Pneumonia in Children"; 2. Age >= 8 years old; 3. Duration of illness <= 5 days

排除标准:

1.已在院外使用四环素类、喹诺酮类抗生素药物治疗;
2.明确使用阿奇霉素治疗72h无反应者;
3.对本研究使用药物过敏者;
4.合并免疫缺陷病或其他肺、肝、肾、心脏等基础疾病者;
5.合并细菌、病毒、真菌等混合感染者;
6.临床表型为危重症型;
7.依从性不佳者或患儿及监护人不同意参加该研究;

Exclusion criteria:

1.Antibiotics such as tetracyclines and quinolones have been administered outside the hospital for treatment;
2.Those who did not respond after 72 hours of treatment with azithromycin should be given further treatment;
3.Those who are allergic to the drugs used in this study;
4.Those with combined immunodeficiency diseases or other underlying diseases such as lung, liver, kidney or heart disorders;
5.Those who have combined infections;
6.Clinical phenotype: critical;
7.Those with poor compliance or the patients and their guardians do not agree to participate in this stud;

采用区组随机化,block 为 4,对符合入组标准的患者按 1:1比例随机分配至两个治疗组。随机化过程由北京儿童医院临床流行病学与循证医学中心统一生成随机编码列表。随机分配采用简单信封分配隐藏方式,为每位受试者指定给药方案。

Randomization Procedure (please state who generates the random number sequence and by what method):

Using block randomization with a block size of 4, patients who met the inclusion criteria were randomly assigned to two treatment groups in a 1:1 ratio. The randomization process was uniformly generated by the Clinical Epidemiology and Evidence-Based Medicine Center of Beijing Children's Hospital and involved a random code list. The random allocation was carried out using a simple envelope allocation method, and a specific treatment plan was assigned to each subject.
