研究目的：

本研究旨在构建基于多学科协作的老年慢病院内外一体化数智药物管理模式，通过整合人工智能、物联网与多学科协同机制，实现以下目标： 理论框架：建立覆盖院内外全场景的闭环管理模型，明确医、药、护、技等多学科协作路径； 技术赋能：开发数智药物管理平台，打通院内外数据壁垒，降低老年患者药物不良事件发生率； 实践验证：通过试点应用验证模式对用药依从性、药物素养、再入院率.  

Objectives of Study：

The purpose of this study is to construct a multi-disciplinary collaboration-based integrated intelligent drug management model for elderly chronic hospitals. By integrating artificial intelligence, Internet of Things and multi-disciplinary collaboration mechanisms, the following objectives are achieved : Theoretical framework : establish a closed-loop management model covering the whole scene inside and outside the hospital, and clarify the multidisciplinary collaboration path of medicine, medicine, nursing and technology. Technology empowerment : develop a digital intelligent drug management platform, break through the data barriers inside and outside the hospital, and reduce the incidence of adverse drug events in elderly patients ; practice verification : through the pilot application verification model for medication compliance, drug literacy, readmission rate.

纳入标准：

1. 年龄>=65岁； 2. 同时患>=2种慢性病（如高血压、糖尿病、冠心病等）； 3. 目前使用>=5种口服药物； 4. 具备基本数字设备使用能力（如智能手机）； 5. 自愿参加本研究。

Inclusion criteria

1. Age >= 65 years; 2. With >= 2 chronic diseases (such as hypertension, diabetes, coronary heart disease, etc.); 3. More than 5 kinds of oral drugs are used; 4. Have the ability to use basic digital devices (such as smart phones); 5. Volunteer to participate in this study.

排除标准：

1. 严重认知障碍、终末期疾病或预期寿命<1年； 2. 拒绝签署知情同意书。

Exclusion criteria：

1. Severe cognitive impairment, end-stage disease or life expectancy < 1 year; 2. Refuse to sign informed consent.
