研究目的：

我科开展此项课题目的是结合FMEA和RCA理论，形成改良型的风险管理方法，以降低行膝关节置换术患者深静脉血栓发生率，提高患者满意度，减少患者住院天数，从而提高护理质量，保证患者安全。对骨科手术患者具有较高的临床应用价值，为预测医疗风险与制定防范策略提供依据。  

Objectives of Study：

The aim of this project in our department is to combine FMEA and RCA theories to form an improved risk management method to reduce the incidence of deep vein thrombosis in patients undergoing knee arthroplasty, to improve patient satisfaction, and to reduce the number of days of hospitalisation of the patients, so as to improve the quality of nursing care and ensure the safety of patients. It has high clinical application value for orthopaedic surgery patients and provides a basis for predicting medical risks and formulating preventive strategies.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）经临床症状、影像学等综合检查明确为膝骨关节炎，诊断标准参考《中国骨关节炎诊疗指南（2021年版）》；（2）具备手术指征；（3）首次进行单侧全膝关节置换术治疗；（4）视听功能正常，可正常沟通交流。

Inclusion criteria

(1) Osteoarthritis of the knee is clearly defined by comprehensive examination of clinical symptoms and imaging, and the diagnostic criteria refer to the 'China Osteoarthritis Diagnostic and Treatment Guidelines (2021 Edition)'; (2) there are indications for surgery; (3) the first time to be treated with unilateral total knee arthroplasty; and (4) the visual and auditory functions are normal, and normal communication is possible.

排除标准：

（1）存在重要脏器严重功能障碍；（2）合并其他影响下肢活动的疾病；（3）凝血功能异常者；（4）术前或既往有血栓史。

Exclusion criteria：

(1) Presence of severe dysfunction of vital organs; (2) Combination of other diseases affecting lower limb activities; (3) Abnormalities in coagulation function; (4) Preoperative or previous history of thrombosis.

应用FMEA结合RCA理论对同类病人的深静脉血栓预防常规流程进行失效模式分析，计算每项失效模式的风险优先数值（RPN），入选RPN值较高的失效模式，使用根本原因分析法（RCA）进行原因分析，制定改进措施，优化现有流程。

FMEA combined with RCA theory was applied to analyse the failure modes of the routine process of DVT prevention for similar patients, calculate the risk priority value (RPN) of each failure mode, and select the failure modes with a higher RPN value, conduct a root cause analysis using root cause analysis (RCA) to formulate improvement measures and optimise the process.
