研究目的：

联合应用经颅多普勒超声（TCD）和近红外光谱仪（NIRS）监测主动脉全弓置换手术（TAR）患者 CPB 停循环至左侧颈总动脉吻合开放期间双侧大脑中动脉（MCA）血流和双侧额部 rSO2，评价主动脉全弓置换（TAR）期间单侧顺行性脑灌注方式（u-SACP）和双侧顺行性脑灌注方式（b-SACP）的效果及其对术后脑功能的影响。  

Objectives of Study：

Transcranial Doppler (TCD) and near-infrared spectroscopy (NIRS) were used to monitor bilateral middle cerebral artery (MCA) blood flow and bilateral frontal rSO2 in patients undergoing total aortic arch replacement (TAR) during the period from CPB circulatory arrest to left common carotid artery anastomosis opening. To evaluate the efficacy of unilateral antegrade cerebral perfusion (u-SACP) and bilateral antegrade cerebral perfusion (b-SACP) during total aortic arch replacement (TAR) and their influence on postoperative brain function.

纳入标准：

纳入标准： 1.在本医疗中心接受全弓置换术的患者 2. 年龄>18 岁，性别不限 3. ASA III 或 IV 级

Inclusion criteria

Inclusion criteria: 1. Patients undergoing total arch replacement at our medical center; 2. Age >18 years old, regardless of gender; 3. ASA Grade III or IV.

排除标准：

排除标准： 1.颞窗闭合或因技术原因不能测及大脑中动脉的患者 2.患者或家属拒绝参加本试验

Exclusion criteria：

Exclusion criteria: 1. Patients with temporal window closed or middle cerebral artery could not be detected due to technical reasons; 2. The patient or family declined to participate in the trial.
