以二元疾病管理理论为指导，构建ICU患者出科准备护理方案，并评价其应用效果  

Objectives of Study：

Guided by the theory of dual disease management, the nursing plan for ICU patients was constructed and its application effect was evaluated

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

患者纳入标准：①年龄18～80岁；②神志清楚，具备语言沟通能力；③首次入住ICU；④ICU住院天数＞3d；⑤知情同意。照顾者纳入标准：①年龄≥18岁，具有血缘或法律关系；②承担患者生活照顾的主要责任和医疗决策；③具有良好的沟通表达及行动能力；④知情同意。

Inclusion criteria

Inclusion criteria: (1) Age 18~80 years old; (2) Be conscious and have verbal communication skills; (3) First ICU admission; (4) The length of ICU stay > 3 days; (5) Informed consent. Inclusion Criteria for Caregivers: (1) Age ≥ 18 years old, with blood or legal relationship; (2) Assume the primary responsibility for the patient's life care and medical decision-making; (3) Have good communication and action skills; (4) Informed consent.

排除标准：

患者排除标准：①无照顾者；②沟通障碍或既往有严重精神病史；③同时参与其他研究；④放弃治疗或转院;⑤未完成研究任务。照顾者排除标准：①患有精神心理疾病；②需同时照顾多名患者；③同时参与其他研究。

Exclusion criteria：

Patient Exclusion Criteria: (1) No caregiver; (2) Communication disorder or previous history of severe mental illness; (3) concurrent participation in other studies; (4) Waiver of treatment or transfer; (5) Failure to complete the research task. Caregiver Exclusion Criteria: (1) Suffering from mental illness; (2) need to take care of multiple patients at the same time; (3) Participate in other studies at the same time.
