研究目的：

一、以本项目所在单位上海市同仁医院作为区域性医疗中心牵头，联合区域卫生管理部门，联动10家社区卫生服务中心，以国家基本公共卫生服务项目——老年人群体检项目作为依托，建立社区老年人群队列。 二、根据队列研究挖掘的健康风险因素及量化模型，结合文献研究、定性访谈，使用边界分析，构建区域老年人主要健康问题集；其次使用意向论证、问卷调研等方法对健康问题进行优先级排序，筛选区域老年人的关键性健康问题，再次在博弈论的视角下，使用米切尔评分和专家咨询方法，建立动态平衡，构建区域老年人关键性健康问题的场景规划和解决路径设计，形成系统的健康管理策略，最后结合长宁区诊疗的实证数据，分析健康策略的可行性和未来走势，提出优化思路。 三、以长宁区的区域老年人为对象，构建以重点疾病为单位的具体化健康管理策略，覆盖三级预防的各个阶段，形成“专科-全科-基层医疗机构-社区和家庭”四级融合的分级诊疗路径、服务内容和执行标准，通过科学规范的应用研究，评价“MOTN”网络式健康管理模式的效果和效益。  

Objectives of Study：

Shanghai Tongren Hospital, where the project is located, as a regional medical center, will take the lead, cooperate with the regional health management department, link 10 community health service centers, and establish a community elderly population cohort based on the national basic public health service project - the elderly group inspection project. According to the health risk factors and quantitative models mined by cohort studies, combined with literature research, qualitative interviews, and boundary analysis, the main health problem sets of the elderly in the region were constructed. Secondly, from the perspective of game theory, Mitchell score and expert consultation methods were used to establish a dynamic balance, and the scenario planning and solution path design of key health problems of the elderly in the region were constructed, so as to form a systematic health management strategy, and finally combined with the empirical data of diagnosis and treatment in Changning District, the feasibility and future trend of the health strategy were analyzed, and the optimization ideas were proposed.

1）>=65岁； 2）已经完成2024年体检； 3）预期能继续参加2025年度体检； 4）无严重身体残疾，能正常交流； 5）自愿参与研究并签署知情同意书，配合度较高。

Inclusion criteria

1) > = 65 years old; 2) Have completed the 2024 physical examination; 3) Expected to continue to participate in the 2025 annual physical examination; 4) No serious physical disability, able to communicate normally; 5) Voluntarily participate in the study and sign the informed consent form, with a high degree of cooperation.

随机序列由一名独立的统计学家负责产生，使用计算机生成随机数，以上海市长宁区江苏、 周家桥、天山路、 仙霞、 程家、 北新泾、 新泾镇共 7 家附属社区卫生服务中心为单位， 采取整群随机的方式，将前4 家社区卫生服务中心作为干预组， 后3 家社区卫生服务中心为对照组。
