研究目的：

（1）在Neuman系统理论和Lazarus压力与应对理论的指导下对肝移植患者健康管理现状进行定量与定性评估分析后，筛选可能影响健康管理过程的影响因素。（2）在Neuman系统理论、Lazarus压力与应对理论以及上游思维的要素指导下，构建基于Neuman系统理论的肝移植患者全程健康管理理论框架。（3）在量性与质性研究结果基础上并结合现有文献和理论框架的指导下，确定肝移植患者全阶段健康管理的要点和路径，构建“上游视角”下肝移植患者全程健康管理模式。（4）根据纵向随访数据，评价“上游视角”下肝移植患者全程健康管理模式的应用效果，并对该模式进行进一步的完善和提高。

Objectives of Study：

(1) under the guidance of Neuman system theory and Lazarus Stress and coping theory, we evaluated the status quo of health management of liver transplantation patients quantitatively and qualitatively, and then screened the influencing factors that might affect the health management process. (2) under the guidance of Neuman system theory, Lazarus Stress and coping theory and upstream thinking, the theoretical framework of whole course health management of liver transplantation patients based on Neuman system theory was constructed. (3) on the basis of quantitative and qualitative research results, and under the guidance of the existing literature and theoretical framework, to determine the key points and paths of the whole-stage health management of liver transplantation patients, to construct the whole course health management model of liver transplantation patients under"Upstream view". (4) according to the longitudinal follow-up data, to evaluate the effect of the whole course health management model of liver transplantation patients under the"Upstream view", and further improve the model

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

1、同意实施肝移植手术并签署手术知情同意书的患者；2、年龄>=18岁；3、无精神、语言障碍等方面的疾病；4、自愿参与本研究。

Inclusion criteria

1. Patients who agree to undergo liver transplantation and sign the informed consent form for surgery; 2. Age>=18 years old; 3. No mental and language disorders; 4. Voluntarily participate in this study.

排除标准：

1、预计短期内可能发生无法避免的死亡的肝移植患者；2、存在严重基础疾病或重要脏器存在严重病变未能进行手术的患者；3、自动出院的肝移植受者；5、接受过其他部位器官移植的肝移植受者。

Exclusion criteria：

1. Liver transplant patients who are expected to die unavoidably in the short term; 2. Patients with serious underlying diseases or serious lesions of important organs that fail to undergo surgery; 3. Liver transplant recipients who are automatically discharged; 5. Liver transplant recipients who have received organ transplantation from other sites.
