研究目的:

(1)本项目主要研究目的是观察经典Byars分期手术和保留远端尿道板的改良Bracka分期手术两种两阶段分期修复手术方法术后出现的并发症,明确阴茎再弯、尿道瘘、龟头裂开、尿道外口狭窄、尿道狭窄、尿道憩室等并发症发病率,探讨改良Bracka分期手术是否可以有更优的外观和良好的功能。 (2)本团队在临床实践中多选用经典Byars分期手术,虽有较高成功率,但在临床实践中发现经典Byars分期手术,难以避免会有尿道憩室的发生。文献报道看来,Bracka手术及其改良手术,能减少尿道憩室发生。本项目将分析比较两种术式的尿道憩室发病率,明确保留远端尿道板的改良Bracka分期手术是否能减少尿道憩室发生的风险。 (3)经典Bracka手术及其改良术式对于原远端尿道板处理,大多是切除或劈开尿道板一直到龟头尖端,插入皮瓣成形新的尿道板。若保留远端尿道板,二期手术时可进行TIP手术方式,增宽远端龟头尿板,卷管成形新尿道。明确在保留远端尿道板的改良Bracka分期手术中,一期手术保留尿道板后是否可以达到简化手术操作、减少术中出血量且缩短手术时间的目的。 (4)尿道下裂的手术修复经过泌尿外科医生的努力探索和技术改进,手术成功率已逐步提高,但因随访数据纷杂,缺少统一和规范量表评估导致目前尚缺乏统一的标准化和规范化的治疗策略。针对此问题,利用客观量表评估,对两种术式术后阴茎外观评分,以便进行有效的比较,对修复尿道下裂的创新手术方法的结果进行平衡评估,以高质量证据形势给将来的临床实践以参考的数据。  

Objectives of Study:

(1) the aim of this study was to observe the postoperative complications of two-stage repair procedures, the classical Byars and the modified Bracka procedure with preservation of the distal urethral plate, to determine the incidence of complications such as penile reflexion, urethral fistula, glans split, urethral orifice stenosis, urethral stricture, urethral diverticulum, etc. , to investigate whether the modified Bracka staging surgery can have better appearance and function. (2) the team chose the classic Byars staging surgery in clinical practice, which had a high success rate, but found that the classic Byars staging surgery in clinical practice, it is difficult to avoid the occurrence of urethral diverticulum. In the literature, Bracka surgery and its modification appear to reduce the incidence of urethral diverticula. This project will analyze and compare the incidence of urethral diverticula between the two procedures to determine whether modified Bracka staging with preservation of the distal urethral plate can reduce the risk of urethral diverticula. (3) the classic Bracka operation and its modification for the treatment of the original distal urethral plate, most of which were resected or split until the tip of the glans, and the skin flap was inserted to form the new urethral plate. If the distal urethral plate is preserved, TIP can be performed during the second stage of surgery to widen the distal turbinate plate and roll up the tube to form a new urethra. Objective: to determine whether preserving the distal urethral plate in one stage of modified Bracka staging surgery can simplify the operation, reduce intraoperative bleeding and shorten the operation time. (4) the surgical repair of hypospadias has been improved by urologists, but the success rate has been gradually increased, the lack of uniform and standardized scales results in the lack of uniform and standardized treatment strategies. In view of this problem, the objective scale is used to evaluate the penis appearance score of the two kinds of operation, so as to compare effectively and balance the results of the innovative operation for repairing hypospadias, high-quality evidence to inform future clinical practice.

纳入标准:

1: 年龄为1—10岁患儿; 2: 符合近端型尿道下裂临床诊断患儿:尿道口位置靠近阴囊且伴有阴茎下弯。 3: 阴茎弯曲度>30°,适合行分期修复术的患儿 4: 无外院手术史或多次手术史 5: 外生殖器局部无明确外伤史 6: 签署知情同意书 7: 患儿性别为男性

Inclusion criteria

1: Children aged 1-10 years 2: children clinically diagnosed with proximal hypospadias: the urethrat opening is located near the scrotum and accompanied by the vagina S.The stem is recurved. 3: The curvature of the penis >30° is suitable for children with staged restoration 4: No surgical history or multiple surgical history in other hospitals 5: No clear history of local external genital trauma 6: Signed informed consent 7: The child is male

排除标准:

1: 以行一期手术修复的患儿 2: 龟头宽度小于1.0cm的患儿 3: 有凝血功能障碍等疾病

Exclusion criteria:
