研究目的：

1.探索可行的农村慢病整合服务提供模式；2.探索支付系统对于整合的作用;3. 论证整合服务效率效果。  

Objectives of Study：

1. To explore possible model of integrated care delivery; 2. to expplore the effect of payment system on integrated devlivery system; 3. to test the effectiveness and efficiency of integrated delivery and payment system.

纳入标准：

Inclusion criteria

排除标准：

1）由于血压/血糖长期稳定从而无须服药患者；2）预期寿命少于两年导致失访可能性过高患者；3）由于外出务工、访亲等原因导致失访可能性过高患者；4）智力受损或行为活动能力低下患者；5）精神状态不稳定或其他无法沟通患者；6）由于个人原因拒绝签订知情同意书的患者。

Exclusion criteria：

1. To be had a stable BP/GLU history (consistently under 120/80 mmHg and 6.5 m mol/L) longer than 1 year and thus would not admit having chronic diseases or refuse to take medicines;
2. To be estimated for a life expectancy less than 2 years due to old age, venerable situations or severe complications such as cerebral infarction or pancreatic cancer, thus would potentially call for apparent extra samples;
3. There is a potential of getting lost in follow-ups with high chances to go out, which was recognized for at least 6 months away yearly during the intervention period for reasons as migration for work, education or kinship-care-seeking;
4. There is a potential of hardly visiting or investigating due to intellectual or activity incompetence;
5. To be with mental damage or communication incapable;
6. Refuse to cooperate due to personal reasons.
