研究目的：

将永久性栓塞剂α-氰基丙烯酸灌注于食管下端和胃底曲张静脉内及胃底和贲门周围曲张静脉丛，分别观察改良经皮经肝食管胃静脉曲张栓塞技术（PTVE）与传统栓塞剂对曲张静脉消失率、急症止血率、复发率、再出血率及患者生存率的影响，并探讨介入栓塞治疗食管胃底静脉曲张破裂出血技术方法。  

Objectives of Study：

To approach the technology in esophageal variceal bleeding management, we performed a modified Percutaneous Transhepatic Variceal Embolization(PTVE) with 2-octyl cyanoacrylate (2-OCA) in the whole lower esophageal and peri or para-esophageal varices, the submucosal varices, and the advertitial plexus of the cardia and fundus were sufficiently obliterated. We compared this modified PTVE with traditional embolotherapy in the eradication of esophageal varices, varices relapse, rebleeding and survival rate.

纳入标准：

Inclusion criteria

排除标准：

1.合并有肝癌或其他部位肿瘤；2.合并有广发门静脉栓塞、严重门静脉海绵样变；3.严重高血压、冠心病、心肺功能不全；4.Ⅱ期以上肝性脑病；明显黄疸，总胆红素在正常3倍以上；5.合并肝肾综合症或慢性肾功能不全；6.明显出血倾向，PT＞25秒；7.任何原因所致严重门静脉插管困难者；8.患者及家属不愿完善术前检查或者不接受术后定期复查者。

Exclusion criteria：

1. association with hepatocellular carcinoma or other malignancy;
2. complete obstruction of the portal vein due to thrombosis;
3. grave hypertension, coronary heart disease, or impaired function of cardiorespiratory;
4. encephalopathy greater than stage II; severe jaundice with serum bilirubin more than 100 mg L-1;
5. presence of hepato-renal syndrome or chronic renal insufficiency;
6.obvious hemorrhagic tendency, PT>25s;
7. The portal vein was failure to venous cannula with kinds of cause;
8. inability or refusal to give the consent to complete preoperative check and postoperative recheck.

1. 对入组患者随机行经皮经肝穿刺门静脉穿刺、插管、造影，将永久性栓塞剂α-氰基丙烯酸经导管注射到食管胃底粘膜下曲张静脉、壁内穿支静脉及食管胃底周围，使曲张静脉及其供血静脉彻底永久栓塞。 2.若患者合并胃后静脉和胃短静脉供血，用相同方法一并栓塞，直至术后造影曲张静脉栓塞完全。 3.拔管前，明胶海绵条或钢圈封闭肝穿刺道，防止穿刺道出血

The patients was performed PTVE by venipuncture, intubation tube, visualization, 2-OCA was then injected continuously until all the varices in lower esophagus and and peri or para-esophageal varices and the gastric fundus and the cardia varices.

干预措施：

1.对入组患者随机行经皮经肝穿刺门静脉穿刺、插管、造影同上，胃冠状静脉或胃后静脉、胃短静脉插管后，先注入无水酒精10～30ml，曲张静脉血流减慢并无门静脉逆流后再注入明胶海绵颗粒（2mm×2mm×1mm）,最后用不锈钢圈堵塞胃冠状静脉或胃后静脉、胃短静脉主干。 2.复查门静脉造影示曲张静脉血流完全阻断后，退管、封闭肝实质穿刺道。

The patients was performed PTVE as the experimental group, vena coronaria ventriculi, vena gastrica posterior or short gastric veins was injected absolute alcohol firstly, then reinject the gelatin sponge, the stainless-steel coils was obstructed the stem of veins lastly.
