研究目的：

甲状腺组织血供丰富，且临近气管，喉返及喉上神经，手术易导致出血、神经损伤、窒息等并发症，目前的电凝设备较少应用于甲状腺切除，仍沿用传统的缝线结扎以保证手术的安全性、可靠性。同时甲状腺肿瘤多见于中青年女性，对手术切口及愈合要求较高，但单纯小切口甲状腺手术往往增加手术难度、手术时间及手术并发症的发生率。LigaSure作为新一代临床止血器械，应用实时反馈技术和智能主机技术，输出双极高频电能，结合血管钳压力，使胶原蛋白和纤维蛋白溶解变性，血管壁融合形成一闭合带，可抵御超过人体正常收缩压三倍的压力。可闭合直径不超过7mm的动静脉，闭合快，无炭化，产生烟雾少，热传导不超过2mm。该系统安全、高效，已较多用于腹腔镜及腹部开放性手术，大大缩短了手术时间，提高了手术效率。本研究试图探讨LigaSure在甲状腺手术中的应用价值。  

Objectives of Study：

Thyroidectomy is considered to be a safe and standardized procedure with a minimal complication rate. Adequate hemostasis is essential for a safe surgical technique to avoid damage to the superior and recurrent laryngeal nerves and parathyroid glands. The clamp-and-tie technique (CTT) is still the most frequently used standard, although it requires several steps which in addition are time-consuming. Several techniques of occluding vessels, besides the classic ligatures, are currently available. An alternative technique is the application of LigaSure vessel-sealing system (VSS) that have proven to be fast, effective and safe for achieving hemostasis in abdominal and pelvic surgery. However, new techniques for quick and safe hemostasis have to be tested in clinically relevance randomized controlled trials before a general recommendation can be given. The present study is designed to compare the outcome of thyroidectomy using the VSS versus CTT by means of randomized controlled trial.

纳入标准：

Inclusion criteria

排除标准：

1.需要行淋巴结清扫术；2.侵犯声门的甲状腺疾病；3.二次甲状腺手术；4.既往颈部手术史；5.有明显的心、肺、脑、肾功能障碍； 6.正在参加其它临床实验者；7.妊娠、哺乳期妇女。

Exclusion criteria：

Malignant disease or highly suspicion for malignancy; Severe psychiatric or neurological disease; Previous history of thyroid or neck surgery; Participation in another intervention-trial; pregnancy.
