研究目的:

探索和完善心内非接触式标测技术指导下个体化消融方法。初步确定不同机制的房颤最适宜的消融线路、不同消融方法的消融终点、左心房或肺静脉不同部位最适宜的消融能量和消融时间。 证实心内非接触式标测技术指导下个体化消融方法在房颤消融成功率、房颤复发率、消融并发症等方面优于环肺静脉左房线性消融术。 拓宽房颤导管消融的适应证。以往房颤导管消融的对象局限于心脏结构正常的阵发性房颤,借助心内非接触式标测技术,将房颤导管消融的对象逐步扩展到持续性房颤、部分瓣膜性心脏病房颤,使房颤群体中更多的患者受益。并最终使其得以更大程度的推广和接受。  

Objectives of Study:

Most ablation strategies for atrial fibrillation consist of a standardized approach of pulmonary vein isolation and/or encirclement with or without additional ablation lines. However, because the pathogenesis of AF is multifactorial, a standardized ablation approach may not be equally effective or efficient in all patients with AF. A predefined, standardized lesion may involve more ablation than necessary in some patients and may be insufficient in others. The hypothes of the study was that the triggers and drivers of AF vary from patient to patient and that it is possible to identify the specific generators of AF by non contact mapping. This study determined the feasibility of an individualized catheter ablation strategy guided by non-contact mapping system, without the use of a standardized lesion set.

纳入标准:

Inclusion criteria

排除标准:

①患有胺碘酮或抗凝剂如华法令的禁忌症;
②左房内径〉55mm;
③充血性心力衰竭:左室射血分数<35%;
④食道心脏超声检查或CT或MRI发现左房血栓;
⑤某些器质性心脏病如扩张性心肌病、冠心病心肌梗塞等;但高血压病和冠心病心绞痛有效控制后可入选;瓣膜性心脏病:原发病未能得到有效矫治或安装了机械瓣;
⑥脑卒中病史;
⑦其他原因引起的心房颤动,如甲亢、酒精或药物中毒等,病因未去除前。

Exclusion criteria:

1. Contraindication to amiodarone therapy or anticoagulation with warfarin;
2. Left atrial diameter >55 mm;
3. Left ventricular ejection fraction <35 percent;
4. Presence of left atrial thrombus on transesophageal echocardiography or CT or MRI;
5. Structural heart disease: Unstable angina, acute or prior myocardial infarction, uncontrolled systemic hypertension, nonischemic cardiomyopathy, uncorrected valvular disease or presence of a mechanical prosthetic valve;
6. History of a cerebrovascular accident;
7. AF secondary to transient or correctable abnormality (hyperthyroidism, alcoholism and drug intoxication etc);
8. Prior attempt at catheter or surgical ablation for atrial fibrillation.

环肺静脉左房线性消融组

在电解剖标测系统指导下(Carto XP和Ensite NavX均可)构建左心房三维构型,分别于距两侧肺静脉口1~2cm处前庭部作环状消融,为预防左房房扑,在左下肺静脉到二尖瓣环间额外加一条消融线。发放射频电流使最大局部双极电图振幅减小≥80%或≤0.1mv。消融终点为肺静脉完全电隔离,即肺静脉-左心房双相阻滞。如肺静脉电隔离完成后房颤未终止,则行体外直流电复律

CARTO or NavX used for left atrial geometry construction. Pulmonary vein encirclement and electrical isolation with additional roof and isthmus ablation lines. Cardioversion is used to restore sinus rhythm if AF still present at the end of the procedure

Ensite array左房及肺静脉前庭部建模。手术时为窦性心律的病人程序刺激诱发房颤。术中记录到房颤后,以2Hz的高通(high pass)滤波设置和自动聚焦设置连续观察时至少10个心房周期的内膜等电位激动,结合虚拟内膜电位,判定其分型及关键传导部位后,在其关键部位进行消融,保持消融部位在肺静脉口外5~10mm,消融部位的选择、扩大和延伸均取决于每次标测的结果,并根据消融后的变化做出步进式调整,直至阻断传导并终止房颤

Ensite array be used for anatomical navigation and activation mapping. Incremental stimulation used to induce AF if it is not present. Stepwise linear ablation guiding by non contact mapping are created until termination and noninducibility of AF.
