研究目的：

老年人是疾病的高发人群，住院率是其他年龄人群2倍，因高龄和患病生活不能自理者达一千多万。中国是在经济较不发达社会背景下，率先进入老年化行列的国家。发达国家的老年病临床研究显示，综合、定期、多学科参加的老年综合评估能显著改善老年患者的生活质量，延长居家生活时间。30年来，美国的大量临床研究证明，老年综合评估系统能降低老年患者的医疗费用、缩短老年患者平均住院日、并通过早期筛查早期干预提高患者对医疗的满意度。它具有费用低廉、操作方便的特点。但在中国，相关的研究和实践明显薄弱。为寻找一个适合中国特色的具有节约型特点的老年疾病临床工作模式，达到减少老年人平均住院日、再入院率，节约医疗卫生资源的目的，特在老年科住院和门诊病人中开展老年综合评估，并根据评估结果调整治疗方案，观察采用综合评估干预对患者预后的影响。

To observe the prognosis of comprehensive geriatric assessment and management in the group of older frailty patients with multiple diseases.

排除标准：

到院后7日内死亡的患者

Exclusion criteria：
