研究目的：

通过RCT研究证实肝脾联合切除术较单纯肝切除可明显提高肝癌合并肝硬化门脉高压症患者的累积生存率和无瘤生存率  

Objectives of Study：

To determine the influence on prognosis of hepatectomy combined with splenectomy for hepatocellular carcinoma patients with cirrhosis

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

（1）术前门静脉主干及一级分支、肝总管及一级分支、肝静脉主干及下腔静脉有癌栓；（2）有肝外转移、淋巴结转移者；（3）病理证实活动性肝炎；或HBV NDA大于1000copy/ml。（4）手术前接受全身化疗，介入治疗等其他辅助治疗的患者。（5）、合并糖尿病、心脏病的患者及其他疾病无法耐受手术者。（6）、梗阻性黄疸者。（7）术后2月内复发者。

Exclusion criteria：

1. Preoperative examinations show tumor thrombus in main portal vein and its first branches, common hepatic duct and its first branches, main hepatic veins, and hepatic veins;
2. Neoplasm metastasis out of liver or in lymph nodes;
3. Pathology shows active hepatitis;
4. Chemotherapy or interventional therapy or any other adjunctive therapy before operation;
5. Patients complicated with diabetes mellitus or heart diseases or any other organ diseases, can't tolerate the surgery;
6. Obstructive jaundice;
7. Postoperative recurrence in 2 months.
