研究目的：

比较在原发性闭角型青光眼临床治疗中的两种治疗策略的有效性、安全性和卫生费用，两种策略分别为：以小梁切除术作为初始治疗的治疗策略，和以激光周边虹膜切除术作为初始治疗的治疗策略。  

Objectives of Study：

To compare the effect, safety and health expenditure of two kinds of treatment strategies in the clinical treatment for PACG. One is Trabeculectomy as the initial treatment, and the other is the laser peripheral iridotomy as initial treatment.

纳入标准：

Inclusion criteria

排除标准：

1.继发性青光眼（如新生血管性，葡萄膜炎，外伤，晶状体膨胀，晶体脱位等），或者急性闭角型青光眼（包括既往有急性发作病史或者存在明确发作体征者）；
2.严重视野损害（MD>20dB）; 或者非青光眼性的视野损害（如视神经炎，视乳头水肿，缺血性视神经病变等，颅脑疾病等），不能获得可靠的视野检查结果；
3.短眼轴患者<20mm; 存在视网膜血管异常或者可能影响视野结果的视网膜疾病，如晚期老年黄斑变性，动脉/静脉阻塞，视网膜脱离，活动性眼内炎或者葡萄膜炎；
4.曾接受结膜和眼内手术，以及受过影响视觉通路的眼外伤患者；1年内可能进行白内障手术的患者；
5.长期使用匹罗卡品，停药3天后瞳孔不能恢复到2.4mm以上的患者；
6.严重的全身性疾病，如中风、肿瘤、长期系统性抗凝（如阿司匹林）或抗肿瘤药物使用患者，肾透析患者；
7.单眼患者；
8.不能或者不愿意按照项目要求进行长期随访的患者；
9.拒绝签署知情同意书。

Exclusion criteria：

1. Secondary glaucoma (such as neovascular glaucoma , uveitis, external injury, lens expansion, etc. or acute angle-closure glaucoma (including patients with past history of acute onset or definite onset signs);
2. Severe visual field damage (MD>20dB) or non-glaucoma visual field damage (such as optic neuritis, papilledema, ischemic optico-neuropathy, cerebral disease, etc) and the reliable visual field examination result can not be obtained;
3. Patient with short axial length <20mm, patients with retinal vessel abnormality or retinal diseases which may effect the visual field examination result such advanced age related macular degeneration, arterial/venous obstruction, retinal detachment, active entophthalmia or uveitis;
4. Patients having received the conjunctiva and intraocular surgeries or with external injury effecting the visual pathway; Patients who may receive the surgery for cataract within 1 year;
5. Patients having taken the Pilocarpine for long time, and the pupil can not restore above 2.4mm three days after withdrawal;
6. Severe systemic diseases such as stroke, tumor as well as patients having taken the systemic anti-coagulants (such as Asprin) or anti-tumor drugs for long time;
7. Patients with single eye;
8. Patients who are unable or unwilling to accept the long term follow-up according to the project requirements;
9. Patients refusing to sign the informed consent.
