研究目的：

1、通过临床实践不断改进和完善，形成腹腔镜下膀胱根治性切除-重建术的标准术式，并成为世界公认的术式； 2、明确腹腔镜膀胱全切除—重建术的临床疗效及并发症，为临床治疗选择手术方式提供有力证据。

Objectives of Study：

1. To improve and standardize techiques of laparoscopic radical cystectomy-bladder reconstruction through clinical practice, and to applicate globally; 2. To assess the oncolgical, functional outcome and complications of laparoscopic radical cystectomy-bladder reconstruction, and provide evidence in choice of treatment.

纳入标准：

Inclusion criteria

排除标准：

1. 证实有远处转移；
2. 尿道断端2cm内有肿瘤，即男性膀胱颈以下有肿瘤，女性膀胱三角区以下有肿瘤；
3. 有膈肌裂孔疝、腹壁疝、腹壁肌松弛、盆底肌松弛等影响腹压的病变；
4. 存在前尿道狭窄；
5. 有明显肠道病变；
6. 既往有小肠切除史；
7. 血象、心、肺、肝、肾功能明显异常，代偿功能不足，或存在严重的全身系统疾病；
8. 女性压力性尿失禁或已作膀胱颈悬吊术；
9. 研究者认为应排除在本研究之外的其他任何情况。

Exclusion criteria：

1. Patient with distant metastasis disease;
2. Tumor in urethral (Tumor locate below bladder neck in male, and trigone of bladder in female);
3. Co-exist disease which may affect intraabdominal pressure (esophageal hiatal hernia, abdominal hernia, abnormal abdominal or pelvic straining);
4. Stricture of anterior urethra;
5. Co-exist intestinal disease;
6. Previous resection of small intestine;
7. Severe heart insufficiency, decompensated pulmonary, liver, renal functons, or other severe systemic disease;
8. Stress incontinence in femal or previous bladder nek suspension;
9. Other situation which was recognized should be excluded by the researcher.
