研究目的：

对比鼻用激素与联合鼻用激素及克拉霉素对中国慢性鼻窦炎伴/不伴鼻息肉患者的疗效  

Objectives of Study：

To compare the effectiveness of claritromycin with Budesonide vs Budesonide for the treatment of CRSw/nNP in China

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

clarithromycin clarithromycin Budesonide Budesonide 

纳入标准：

Inclusion criteria

排除标准：

1. 受试者对克拉霉素及局部糖皮质激素鼻喷剂过敏者；
2. 受试者有下列疾病囊性纤维化，先天性纤毛运动障碍，阿斯匹林不耐受或哮喘，非侵袭性真菌球和侵袭性真菌病，系统性血管炎和肉芽肿性疾病，肿瘤患者，免疫缺陷，过敏性真菌性鼻窦炎和鼻息肉病；
3. 试验前四周内局部及全身使用其它治疗CRS的药物，例如糖皮质激素、大环内酯类药物等；或其它治疗方式，例如鼻腔冲洗等；或正在接受脱敏治疗；
4. 受试者合并哮喘或其他自身免疫性疾病，正长期通过鼻、口腔吸入或者系统给药途径，接受糖皮质激素、白三烯抑制剂及茶碱类药物其中任何一种药物治疗；
5. 受试者正罹患上呼吸道感染；
6. 临床上有严重的代谢、心血管、免疫、神经、血液、消化、脑血管或呼吸系统疾病，或研究员认为任何干扰研究结果的评估或影响受试者安全的疾病；
7. 目前正在参加其他临床研究或30天以内参加过其他临床研究者，或直接参与此研究的工作人员；
8. 具有精神病史、对抗性格、不良动机、多疑或其他情感或智力问题，可能影响参与研究的知情有效性；
9. 妊娠或哺乳期妇女。

Exclusion criteria：

1. Allergy to clarithromycin or topical sterouid spray;
2. With any one of the following diseases: cystic fibrosis based on positive sweat test or DNA alleles; gross immunodeficiency (congenital or acquired); congenital mucociliary problems eg primary ciliary dyskinesia (PCD); non-invasive fungal balls and invasive fungal disease; systemic vasculitis and granulomatous diseases; cocaine abuse; neoplasia;
3. Treated with other CRS medicines 4 weeks before inclusion, such as macrolides or steroids; or other treatment, such as nasal lavage or desensitive treatment;
4. Comorbidity with asthma or other gross immunodeficiency, using steroid through the nose or inhalation or systemically;
5. With upper respiratory tract infection;
6. With severe systemic diseases affecting the cardiovascular, metabolic, immunology, neurology, hematology, cerebrovascular or respiratory system;
7. Participate in other clinical researches within the lastest 30 days or the reseacher themselves;
8. With history of psychic disease, or mental problems;
9. Pregnant or breast-feeding females.
