研究目的：

评估肩袖修复术后0.2%罗哌卡因连续肌间沟臂丛阻滞的最低有效背景剂量和95%患者有效背景剂量。

Objectives of Study：

To evaluate the minimum effective background infusion rate and the effective background infusion rate in 95% of patients of ropivacaine 0.2% for continuous interscalene brachial plexus block that would effectively relieve pain after rotator cuff repair surgery.

药物成份或治疗方案详述：

纳入标准：

Inclusion criteria

排除标准：

BMI≥35、血栓栓塞疾病史、外周神经病、外周神经阻滞的禁忌症（如穿刺部位感染、凝血功能障碍、严重贫血等）、严重限制性或阻塞性肺部疾患、长期使用镇痛药物、不能理解患者自控镇痛装置、拒绝接受神经阻滞的患者。

Exclusion criteria：

1. Patient refusal of interscalene block;
2. Body mass index>=35;
3. Thromboembolism, neuropathies, contraindication for peripheral nerve block (infection at the injection site, coagulation disorders, severe anemia, for example), severe respiratory disease, chronic opioid use, mental illness.

干预措施：	随访若NRPS＜4，则此患者镇痛成功，下一个患者背景剂量降低0.5ml/h。无论在哪一时间点评估NRPS≥4时，肌间沟置管回抽无血液推注1%利多卡因10ml。若NRPS降低，则此患者背景剂量设置过低，给予补救镇痛药，下一患者背景剂量增加0.5ml/h。	干预措施代码：
Intervention：	If the previous patient had successful pain relieve with NRPS<4 during the 48 hours postoperative period, the back ground infusion rate of the next patient was reduced by 0.5ml/h. If the NRPS≥4, a bolus of 10ml lidocaine (1%) was injected through the catheter.	Intervention code：

干预措施：	若NRPS不降低，说明导管位置不佳，撤除此患者肌间沟置管，改用静脉PCA，下一患者背景剂量不变	干预措施代码：
Intervention：	If the NRPS remained more than 3, the case was regarded as technical failure, then the next patient received the same dose. If the NRPS reduced, the case was regarded as having satisfactory catheter placement but an unsuccessful pain relieve, the back ground infusion rate of the next patient was increased by 0.5ml/h.
