研究目的：

在中国，慢性乙型肝炎是导致肝衰竭的主要病因，本研究的目的是评价甲基强的松龙针大剂量（240mg/d）、小剂量（40mg/d）两个剂量对阻断重症化倾向的慢性乙型肝炎进展为慢加急性肝衰竭的有效性和安全性。

Objectives of Study：

Chronic infection with the hepatitis B virus is a major risk factor for development of liver failure in China. The aim is to evaluate clinical efficacy and safety of high- dose glucocorticoid (240mg/d) and low-dose glucocorticoid (40mg/d) in the treatment of patients with severe acute exacerbation of chronic hepatitis.

纳入标准：

Inclusion criteria

排除标准：

（1）使用核苷（酸）类似物后产生耐药患者；（2）正接受化疗和免疫抑制剂治疗的患者；（3）重叠HEV或HAV感染患者；（4）合并自身免疫性肝病患者（包括AIH、PBC和PSC）；（5）合并HCV感染患者；（6）合并HIV感染患者；（7）原发性肝细胞癌患者；（8）合并妊娠患者；（9）存在活动性消化性溃疡等其它糖皮质激素使用禁忌证患者；（10）存在其他心、脑、肾等重要脏器严重疾病影响短期内疗效判断者。

Exclusion criteria：

1. The virus that was detected from these patients had resistance to nucleoside or nucleotide;
2. The patients were receiving chemotherapy and immunosuppressive therapy;
3. Concurrent viral infection,including HAV,HEV;
4. Combined with autoimmune liver disease (including AIH, PBC and PSC);
5. Combined with HCV-infected patients;
6. Combined with HIV infected patients;
7. Patients with primary hepatocellular carcinoma;
8. Pregnancy patients;
9. Existence of active peptic ulcer and other glucocorticoid use in patients with contraindications;
10. Patients with serious diseases of heart, brain, kidney and other vital organs that influence the judgement of the efficacy in short term.
