研究目的：

本项目将通过对“十一五”期间入组的重型乙型肝炎（肝衰竭）存活患者做进一步随访，并研究和新建基于统一诊断标准的 2000 例多中心、大样本、部分随机对照的重型乙型肝炎患者研究队列，进一步优化重型乙型肝炎（肝衰竭）内科综合治疗方案、基于随机、双盲、对照的抗病毒治疗新方案、人工肝个体化治疗新方案；并以此为基础建立一套标准化的且能在全国推广、显著降低病死率的重型乙型肝炎治疗的新方案，使重型乙型肝炎（肝衰竭）病死率由“十一五”的 45% 左右下降至 35% 。 在研究过程中优化重型乙型肝炎（肝衰竭）生物标本库、数据库和诊治网络平台，实现中心实验室统一检测。同时创建评估重型乙型肝炎（肝衰竭）严重程度和预后的评分标准，完善“十一五”期间创建的治疗线路图，创建重型乙型肝炎（肝衰竭）诊治的临床路径，并向全国推广。  

Objectives of Study：

This study is a multi-centerred, partial-randomized controlled cohort study. It recruits more than 2,000 patients who survived from HBV-related severe hepatitis (liver failure) in the Eleventh-Five Year Project throughout the country. The aim of the study is establish an optimized treatment strategy with comprehensive internal medicine approaches against HBV-related severe hepatitis. Based on a double-blinded randomized controlled clinical trial, the new treatment strategy should include antiviral treatment andaritifical liver support system. With the introduction of this new treatment strategy, the morbidity of HBV-related severe hepatitis are expected to decline from 45% to 35%. At the same time, the construction of HBV-related severe hepatitis (liver failure) library are anticipated to upgrade the roadmap in the eleventh-five year, generate a clinical route for the disease, andultimately further studies in the field.

纳入标准：

Inclusion criteria

排除标准：

A. 急性妊娠脂肪肝；
B. 药物性肝损伤；
C. 酒精性肝病(每日饮酒女性超过20g，男性超过40g，5年以上);
D. 自身免疫性肝病；
E. 先天性代谢性肝病，如Wilson病，血色病等;
F. 移植术后乙肝复发导致重型肝炎者；
G. 合并其他病毒现症感染者：抗-HAV IgM、抗-HCV、抗-HEV IgM、抗-CMV IgM、抗-EBV IgM阳性者以及抗HIV抗体任何一项阳性；
H. 合并严重的基础疾病，包括未被控制的有临床意义的心脏、肺、肾脏、消化、神经、精神疾病、免疫性疾病、代谢性疾病、恶性肿瘤等。

Exclusion criteria：

1. Acute fatty liver of pregnance;
2. Drug-induced liver injury;
3. Alcoholic liver disease (daily alcohol intake >=20g in female and>=40g in male,);
4. Autoimmune liver disease;
5. Congenital metabolic liver disease, eg. Wilsons disease, hemachromatosis;
6. HBV relapsers post-liver transplantation;
7. Coinfection with other viruses, any of the antibodies positive: anti-HAV IgM, anti-HCV, anti-HEV IgM, anti-CMV IgM, anti-EBV IgM andanti-HIV;
8. Severe comorbidities with clinical significance, including uncontrolled cardiac, pulmonary, nephritic, digestive, nerve or psychiastric, auto-immune, metabolic andmaglinant diseases.
