研究目的：

初步建立早产的预警模型，以评价孕妇的早产易患性，识别早产高危因素，对早产高危人群进行有效的系统管理，从而有助于对早产的高危孕妇采取更有针对性的预防和治疗措施，有效降低早产风险，从而有助于减少早产儿死亡及并发症的发生，提高新生儿存活率。 具体目标： 1. 采用高通量转录组学分析技术，通过对孕妇外周血有核细胞转录组检测，在全基因组范围内建立代表母体反应的基因表达及转录调控动态图谱，探索引发早产的分子机理。 2. 在转录组学的基础上，联合人口统计学及临床危险因素指标、物理指标（如超声宫颈评价）、生化指标（如阴道分泌物中的胎儿纤维连接蛋白），初步建立早产的多种预警模型。 3. 通过对多种预警模型的评价，初步探索适合我国北京地区的灵敏、简便、无创的早产风险预测模型。  

Objectives of Study：

It aims to establish preterm warning model, in order to evaluate the susceptibility of preterm labor and to identity pregnant women with high risk and provide effective management, so as to be contributive to reduce the occurrence of severe complications and premature deaths, improve life quality. Specific objectives: 1. To explore the molecular mechanism of preterm labor by the detection of mononuclear cells transcriptome in peripheral blood of pregnant women; 2. To initially established several preterm warning model with the different combination of transcriptome, demographic and clinical risk factors, physical indicators (such as ultrasound cervical evaluation) and biochemical parameters (such as fetal fibronectin, fFN); 3. To evaluate preterm warning model above and select the model most suitable for Beijing, China.

纳入标准：

Inclusion criteria

排除标准：

1）因胎儿畸形、胎死宫内或因计划外妊娠引产；
2）因外伤等明确原因的意外引起的早产；
3）双胎或多胎；
4）吸烟、酗酒或吸毒者（早产组、足月对照组指孕前1个月内、孕期）；
5）患有自身免疫性疾病如类风湿关节炎、系统性红斑狼疮等；
6）伴有慢性肝、肾等重要脏器实质性病变者；
7）合并严重急性或慢性疾病，如心脏病（心功能Ⅲ级及以上）、糖尿病、严重甲状腺功能亢进或减退等；
8）合并急、慢性感染者，如呼吸道感染、关节炎、活动性病毒性肝炎等；
9）患有先天遗传性疾病；
10）合并有精神疾患。
11）特殊用药史。

Exclusion criteria：

1) induction due to fetal malformations, fetal death or unplanned pregnancy;
2) preterm birth due to explicit accident, such as trauma;
3) twins or multiple births;
4) smoking, drinking or drug addicts ( one month before pregnancy, and during pregnancy);
5) People with autoimmune diseases such as rheumatoid arthritis, systemic lupus erythematosus;
6) complicated with chronic disease of liver, kidney or other vital organs;
7) with severe acute or chronic diseases, such as heart disease (heart function III grade and above), diabetes, severe thyroid function hyperthyroidism or hypothyroidism, etc.;
8) with impatient, chronic infections, such as respiratory infections, arthritis, active viral hepatitis;
9) suffering from congenital diseases;
10) associated with mental disorders;
11) special medication history.
