研究目的：

老年患者的诊疗需要以患者为中心，除了疾病诊疗，更重要的是维护内在功能，营养治疗与康复训练成为全人治疗的重要组成部分。目前，各家医院的营养评估-干预流程不统一。这份调查表就是为了了解上海综合医院老年科患者的营养治疗情况，包括对于营养状态的评估，营养治疗和对临床结局的影响，目的是提升老年科医师在营养治疗方面的知识水平和实践能力，规范营养不良的管理流程。

Objectives of Study：

The diagnosis and treatment of elderly patients should be patient-centered. In addition to disease diagnosis and treatment, it is more important to maintain internal functions. Nutrition treatment and rehabilitation training have become an important part of whole-person treatment. At present, the nutrition-intervention process in different hospitals is not uniform. The purpose of this questionnaire is to understand the nutritional treatment of geriatrics patients in Shanghai, including the assessment of nutritional status, nutritional treatment and the impact on clinical outcomes. The purpose is to improve the knowledge level and practical ability of geriatrics in nutritional treatment and standardize the management process of malnutrition.

纳入标准：

Inclusion criteria

排除标准：

1. 终末期患者（预计生存期≤3个月），
2. 住院期间转入其他科室；
3. 有营养治疗禁忌症（如血流动力学不稳定）；
4. 已参加其他营养研究项目。

Exclusion criteria：

1. Patients in the end stage (estimated survival <= 3 months);
2. Patients transferred to other departments during hospitalization;
3. Patients with nutritional contraindications (such as hemodynamic instability);
4. Subjects who have participated in other nutrition research projects.
