研究目的：

将超声引导下肋间神经阻滞复合全麻用于上腹部手术，同时与术后吗啡静脉自控镇痛泵形成多模式术后镇痛相结合，旨在观察其减少围术期阿片类药物用量，降低术中、术后患者应激反应，减少术中、术后并发症，促进患者术后快速康复，以及缩短住院时间，降低住院费用方面的临床数据，为上腹部手术的麻醉找到一种最佳麻醉方式提供可靠依据。  

Objectives of Study：

Ultrasound-guided intercostal nerve block combined with general anesthesia for upper abdominal surgery, combined with postoperative morphine intravenous self-controlled analgesia pump to form a multi-mode postoperative analgesia, aiming to observe its reduction of perioperative opioid dosage , Reduce the stress response of patients during and after operation, reduce complications during and after operation, promote rapid recovery of patients after operation, shorten the length of hospitalization, reduce the clinical data of hospitalization costs, and find a kind of anesthesia for upper abdominal surgery The best anesthesia method provides a reliable basis.

药物成份或治疗方案详述：

气管插管后，配置药物0.375%罗??卡因+吗啡2mg+地塞米松10mg+生理盐水至20ml。采用便携式超声仪和高频（12MHz）直线探头，将探头与肋骨垂直放置，即可显示肋间神经的横截面超声图。肋骨下缘和胸膜的成角处即为肋间神经的位置。根据手术切口，探头放置在相应的肋间腋中线下方1-2cm处，探头下外侧旁开0.5 cm，平面内技术进针，看到针尖到达肋骨下缘和胸膜成角处，即胸膜外、肋骨下的阴影区域时，回抽注射器无回血，可注入随机配置好的药物3-5ml，可见局麻药液的明显扩散以及相应位置的胸膜下陷。 

Description for medicine or protocol of treatment in detail：

After tracheal intubation, configure the medicine 0.375% rocaine + morphine 2mg + dexamethasone 10mg + saline to 20ml. Using a portable ultrasound system and a high-frequency (12MHz) linear probe, place the probe perpendicular to the ribs to display the cross-sectional ultrasound image of the intercostal nerve. The angle between the lower edge of the ribs and the pleura is the location of the intercostal nerve. According to the surgical incision, the probe is placed 1-2cm below the mid-axillary line of the corresponding intercostal space. The probe is opened 0.5 cm below the lateral side. The needle is inserted in the plane technology. It is seen that the needle tip reaches the lower edge of the ribs and the pleura angle, namely the extrapleural In the shadow area under the ribs, withdraw the syringe without returning blood. You can inject 3-5ml of the randomly configured medicine. The obvious diffusion of the local anesthetic solution and the pleural depression in the corresponding position can be seen. 

纳入标准：

Inclusion criteria

排除标准：

局麻药物过敏者；内分泌紊乱疾病者；精神疾病患者；凝血功能障碍；孕妇或哺乳期妇女；病态肥胖者；严重心、肺部疾病患者；穿刺点感染；免疫性疾病患者；研究者从医学角度考虑受试者有必要中止试验；患者自己要求停止试验。

Exclusion criteria：

1. Patients allergic to local anesthetics;
2. Endocrine disorders;
3. Patients with mental illness;
4. Coagulation dysfunction;
5. Pregnant or lactating women;
6. Morbidly obese; patients with severe heart and lung diseases;
7. Puncture site infections; patients with immune diseases;
8. From a medical point of view, it is necessary for the subject to suspend the test;
9. The patient himself requests to stop the test.
