研究目的：

评价在择期胸腔镜手术单肺通气的患者单肺通气预处理对其对脑保护的作用。 研究指标： 1.主要研究指标 ①单肺通气预处理对患者围术期脑氧饱和度及动脉氧分压的影响。 ②单肺通气预处理对患者术后认知功能的影响。 ③单肺通气预处理对患者血清神经元特异性烯醇化酶(neuron- specificenolase,NSE)浓度,及S100β蛋白浓度的影响。 2.次要研究指标 ①单肺通气预处理对患者拔管时间的影响 ②观察单肺通气预处理对单肺通气时肺内分流的影响。  

Objectives of Study：

o evaluate the effect of single-lung ventilation preconditioning on brain protection in patients undergoing one-lung ventilation during elective thoracoscopic surgery. Research indicators: 1. Main research indicators ①The effect of preconditioning for one-lung ventilation on cerebral oxygen saturation and arterial oxygen partial pressure during perioperative period. ②The effect of preconditioning for one-lung ventilation on the postoperative cognitive function of patients. ③The effect of preconditioning for one-lung ventilation on the serum neuron-specific enolase (NSE) concentration and S100β protein concentration. 2. Secondary research indicators ①The influence of one-lung ventilation preconditioning on the patient's extubation time ② To observe the effect of preconditioning for one-lung ventilation on intrapulmonary shunt during one-lung ventilation.

纳入标准：

Inclusion criteria

排除标准：

1）拒绝参加本研究；
2）急诊手术；
3）肺功能检查为中重度肺通气和换气功能障碍者；
4）曾行开胸手术病史；
5）严重认知功能损害，如昏迷、痴呆及精神分裂症病史；
6）术中出现严重低氧血症（SPO2<85%），观察3min无改善则排除；
7）ASA III级及以上；
8）严重脑血管疾病者；
9）双侧颈动脉存在狭窄及斑块者。

Exclusion criteria：

1. Refusal to participate in this study;
2. Emergency surgery;
3. Pulmonary function test with moderate to severe lung ventilation and ventilation dysfunction;
4. History of thoracotomy;
5. Severe cognitive impairment, such as coma and dementia And history of schizophrenia;
6. Severe hypoxemia (SPO2<85%) occurred during the operation, and no improvement after observation for 3 minutes will be ruled out;
7. ASA III and above;
8. severe cerebrovascular disease;
9. bilateral Carotid artery stenosis and plaque.
