研究目的：

通过随机、开放、平行对照研究，观察普通氧疗、HFNC 及 NIV对 COPD 合并不同程度 II 型呼吸衰竭的疗效，并使用床旁超声指导 HFNC 设置，建立以 HFNC 为中心的呼吸支持规范化流程，提高 COPD 合并呼吸衰竭患者呼吸支持的规范性和有效性。  

Objectives of Study：

Through randomized, open, parallel controlled studies, observe the effects of ordinary oxygen therapy, HFNC and NIV on COPD combined with different degrees of type II respiratory failure, and use bedside ultrasound to guide HFNC settings, establish a standardized process of respiratory support centered on HFNC, and improve Standardization and effectiveness of respiratory support in patients with COPD and respiratory failure.

纳入标准：

Inclusion criteria

排除标准：

1．需紧急气管插管者；
2．不能获得知情同意；
3．存在 NIV 禁忌者：口面部创伤、痰多排痰能力差、血流动力学不稳定等；
4．短期预后不良者，7 天内死亡风险大者，正在进行姑息性治疗；
5．严重心、脑、肝、肾器官功能衰竭；
6．出现 PIC 窗时咳嗽反射极弱或咳痰无力（项目一）；
7. 合并气胸、咯血、重症肺炎者；
8. 不能获得知情同意者。

Exclusion criteria：

1. Those who need emergency tracheal intubation;
2． Unable to obtain informed consent;
3. Those who have NIV contraindications: oral and facial trauma, poor sputum excretion ability, unstable hemodynamics, etc.;
4. Those with a poor short-term prognosis and those with a high risk of death within 7 days are undergoing palliative treatment;
5. Severe heart, brain, liver, and kidney organ failure;
6. When the PIC window appears, the cough reflex is extremely weak or the expectoration is weak (item 1);
7. Those with pneumothorax, hemoptysis, and severe pneumonia;
8. Those who cannot obtain informed consent.

根据既往研究（BMJ 2003;326:185），我们预计 NIV 治疗 AECOPD 失败率（气管插管及不耐受）约 22%。根据此前关于呼吸衰竭的研究，HFNC 与 NIV 治疗失败率之间的绝对差值为 4%-12%, HFNC 与 NCO 治疗失败率之间的绝对差值为15%-20%。根据既往研究（JAMA. 2016; 316(15):1565-1574.），我们将非劣效界值定为9%。为了在单侧a为0.025时得到80%的统计学把握度，采用 PASS 软件计算，在考虑 5%脱落率的前提下，共需要 190 名受试者（项目一、二各 70 例，项目三 50 例）。 工作人员采用简单随机程序（计算机生成的随机数字）按 1:1 的比例将受试者随机分配到两个治疗组。通过不透明信封来隐匿分配，这些信封以10个为一组的形式保存，HFNC 和 NIV/常规氧疗各 5
