研究目的：

评估验证集教育、运动与康复训练为一体的综合防控干预措施对老年功能下降患者日常生活活动能力、移动平衡能力及其他功能状态的作用

Objectives of Study：

Objective to evaluate and verify the effect of comprehensive prevention and control intervention including education, exercise and rehabilitation training on activities of daily living, mobile balance and other functional status of elderly patients with functional decline.

纳入标准：

Inclusion criteria

排除标准：

1.不能配合完成评估及干预：极严重认知功能减退（全面衰退量表，第七级）、瘫痪等；
2.评估及干预过程中存在高风险者：疾病终末期；严重心肺肾等重要脏器功能不全；不受控制的严重心律失常；急性肺动脉栓塞；近期心肌梗塞、不稳定型心绞痛；近期大手术，或过去3个月发生肢体骨折；
3.依从性差，不易随访，不愿签署知情同意书者；
4.研究者认为需排除的其他状况或事件者。

Exclusion criteria：

1. Unable to cooperate to complete assessment and intervention: extremely severe cognitive impairment (global decline scale, level 7), paralysis, etc;
2. Patients with high risk in the process of assessment and intervention: end stage of disease; severe dysfunction of important organs such as heart, lung and kidney; uncontrolled serious arrhythmia; acute pulmonary embolism; recent myocardial infarction and unstable angina pectoris; recent major surgery or limb fracture in the past three months; and;
3. Poor compliance, difficult to follow-up and unwilling to sign informed consent;
4. Other conditions or events that the researcher thinks should be excluded.
