研究目的：

1. 了解深圳市老年人主要疾病及其危险因素的流行现状。 2. 阐明影响老年人健康的主要疾病的发展趋势及不良结局（死亡、失能、残疾等）的发生率，并分析影响其发展和预后的危险因素。 3. 探讨高血压、糖尿病、高血脂等患者在不同共病模式中的相关指标控制水平与健康结局（死亡、失能、残疾等）的关系，为制定老年共病患者相应指标的控制标准提供参考。 4. 探索遗传基因多态性、环境污染物暴露以及多种生活方式对慢性病发生发展的影响，分析环境、生活方式与遗传因素对慢性病风险的交互作用，为筛选高危人群进行针对性干预提供依据。 5.探索患者多重用药对慢性病控制水平及健康结局的影响，为制定老年人的用药规范提供依据。 6. 建立常见慢性病发生、发展及结局事件的风险预测模型。 7. 建立老年人健康评价指标体系。  

Objectives of Study：

1. Understand the prevalence of major diseases and risk factors for the elderly in Shenzhen. 2. Clarify the development trend of major diseases that affect the health of the elderly and the incidence of adverse outcomes (death, disability, disability, etc.), and analyze the risk factors that affect their development and prognosis. 3. Explore the relationship between the control level of related indicators in different multimorbidity patterns and health outcomes (death, disability, disability, etc.) of patients with hypertension, diabetes, hyperlipidemia, etc., to provide a reference for formulating the control standards of corresponding indicators for elderly patients with multimorbidity. 4. Explore the influence of genetic polymorphism, environmental pollutant exposure, and multiple lifestyles on the occurrence and development of chronic diseases, analyze the interaction of environment, lifestyle and genetic factors on chronic disease risk, and provide basis for screening high-risk groups for targeted intervention. 5. Explore the effects of patients' multiple medications on chronic disease control and health outcomes, and provide a basis for formulating medication guidelines for the elderly. 6. Establish risk prediction models for the occurrence, development and outcome of common chronic diseases. 7. Establish a health evaluation index system for the elderly.

纳入标准：

Inclusion criteria

排除标准：

认知或意识障碍不清、阿尔兹海默症患者或严重精神障碍患者；因听力、视力障碍无法正常沟通的老年人者；卧床失能老人；5年内可能离开深圳居住者；不愿签署知情同意书或不愿参加相关问卷、检查或随访的老人。

Exclusion criteria：

(1) elderly with unclear cognitive or consciousness impairment, Alzheimers disease or severe mental disorders;
(2) cannot communicate normally due to hearing or visual impairment;
(3) Bedridden disabled elderly;
(4) may leave Shenzhen within 5 years;
(5) unwilling to sign informed consent or participate in relevant questionnaires, examinations or follow-ups.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据采集： 问卷调查信息由被培训的调查员收集，在调查前由项目组统一组织进行培训。 体格信息由各社康中心具备执业资格的医师进行测量，所用到的血压计、身高体重仪等均符合国家标准，测量前应对仪器进行校正。生物样本的采集由具备护师资格证的护理工作人员操作，同一指标由专人负责测量。 数据管理： 所有调查及生物样本检测结果存储于“深圳市社区健康服务信息系统”，项目组成立数据管理小组，由专人负责数据的存储及传输。对管理小组进行培训，内容包括系统操作、设备终端加密技术、数据安全存储及传输措施等。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data collection: Questionnaire survey information is collected by the trained investigators, and the project team conducts unified training before the survey. Physical information is measured by qualified physicians in each social health center. The blood pressure monitors, height and weight meters, etc. used are in compliance with national standards, and the instruments should be calibrated before measurement. The collection of biological samples is operated by nursing staff with a nurse qualification certificate, and the same indicator is measured by a dedicated person. Data management: All survey and biological sample test results are stored in the "Shenzhen Community Health Service Information System". The project team has established a data management team, and a dedicated person is responsible for data storage and transmission. The management team receives training, including system operation, equipment terminal encryption technology, data security storage and transmission measures, etc.
