研究目的：

老年糖尿病肌少症严重影响患者的生活质量及临床结局。我们在前期的研究中发现，老年糖尿病患者肌少症发生率高，蛋白质摄入不足是老年糖尿病患者肌少症发生的独立危险因素。本研究拟首次通过带抗阻运动联合补充乳清蛋白，探讨干预对老年2型糖尿病伴肌少症患者身体成分、胰岛素抵抗的影响，同时观察对受试者认知功能、生活质量、临床结局的影响，为安全有效且易于实施的干预方式提供理论依据。  

Objectives of Study：

Elderly diabetic sarcopenia seriously affects the quality of life and clinical outcome of patients. In our previous studies, we found that the incidence of sarcopenia in elderly diabetic patients was high, and insufficient protein intake was an independent risk factor for sarcopenia in elderly diabetic patients. This study intends to explore the effects of the intervention on body composition and insulin resistance in elderly patients with type 2 diabetes mellitus with sarcopenia through resistance exercise combined with whey protein supplementation, and meanwhile observe the effects on the quality of life and clinical outcomes of the subjects, so as to provide a theoretical basis for a safe and effective intervention.

纳入标准：

Inclusion criteria

排除标准：

1.3个月内合并酮症、高渗状态或其他严重应激情况；
2.严重肝肾功能不全（血肌酐>正常上限2倍，ALT>正常上限3倍）；
3.合并严重糖尿病慢性并发症（心功能不全、糖尿病肾病大量蛋白尿、糖尿病眼底病变四期以上或激光治疗后）；
4.慢性胃肠系统严重疾病（例如消化系统肿瘤，消化性溃疡，消化道出血，慢性腹泻，顽固性便秘、肠道功能衰竭、急慢性肠炎、胃肠道梗阻等）；
5.合并严重抑郁、精神分裂的精神疾病；
6.身体残疾；
7.患有骨关节、肌肉、神经系统疾病；
8.严重骨质疏松者；
9.严重心脑血管疾病 (心功能不全、不稳定性心绞痛、严重心率不齐、严重高血压、一过性脑缺血发作)；
10.严重阻塞性肺疾病、哮喘；
11.平日服用乳清蛋白粉者；
12.血常规提示血小板小于100×10^9/L。

Exclusion criteria：

1. Within months, ketosis, hyperosmolar state or other severe stress occurred;
2. Severe hepatic and renal insufficiency;
3. Chronic complications of severe diabetes (cardiac insufficiency, diabetic nephropathy, diabetic fundus stage 4 or above or after laser treatment);
4. Chronic serious diseases of gastrointestinal system (such as digestive system tumor, peptic ulcer, gastrointestinal bleeding, chronic diarrhea, intractable constipation, intestinal failure, acute and chronic enteritis, gastrointestinal obstruction, etc.);
5. Mental diseases with severe depression and schizophrenia;
6. Physical disability;
7. Suffers from diseases of the bone, joint, muscle and nervous system;
8. Severe osteoporosis;
9. Severe cardiovascular and cerebrovascular diseases (cardiac insufficiency, unstable angina pectoris, severe arrhythmia, severe hypertension, transient ischemic attack);
10. Severe obstructive pulmonary disease, asthma;
11. People who take whey protein powder on weekdays;
12. Blood routine indicated that platelets were less than 100 x 10^9/L.
