研究目的：

目的是评估使用数据可视化的心血管风险评估APP对患者治疗依从性、血压变化、心血管风险评分的影响。此外，通过问卷调查的目的是健康素养、依从性、患者赋权和风险沟通方面的变化是否与血压、靶器官损伤和/或生活方式的选择有关。通过定性访谈，将调查该工具的使用是否会增强动机，增加风险意识或产生不必要的影响，例如引起患者焦虑。  

Objectives of Study：

The purpose is to evaluate the impact of the cardiovascular risk assessment APP using data visualization on patient treatment compliance, blood pressure changes, and cardiovascular risk score. In addition, the purpose of the questionnaire survey is whether changes in health literacy, compliance, patient empowerment, and risk communication are related to blood pressure, target organ damage, and/or lifestyle choices. Through qualitative interviews, it will be investigated whether the use of the tool will increase motivation, increase risk awareness or produce unnecessary effects, such as causing patient anxiety.

纳入标准：

Inclusion criteria

排除标准：

1）高血压急症；
2）急性心肌梗死（＜3月）和脑卒中（＜3月）急性期；
3）孕妇、哺乳期妇女；
4）不易配合（智力、听力、肢体活动明显障碍）；
5）合并严重疾病，预期寿命不足一年；
6）医务人员。

Exclusion criteria：

1. Patients with hypertension emergencies;
2. Patients with acute myocardial infarction (< 3 months) and acute stroke (< 3 months);
3. Pregnant women and lactating women;
4. Patients who are not easy to cooperate with (intelligence, hearing and physical activities are obviously impaired);
5. The life expectancy of patients with severe diseases is less than one year;
6. Medical staff.

本研究为整群随机对照研究。入选的3个社区的随机数由四川大学华西医院专业统计人员产生，用SAS 9.4软件产生随机序列并按2:1将社区整群随机分为干预组和对照组。

This study is a cluster randomized controlled study. Random Numbers of 3 communities selected by each sub-center are generated by professional statisticians of West China Hospital, Sichuan University. Random sequences are generated using SAS 9.4 software.
