研究目的：

1.将智能化CGA和康复干预技术推广到基层，为更多老人提供早期评估和健康生活方式指导，使CGA应用前移，实现预防重于治疗的目标。 2.优化住院、门诊、社区不同老年人群的CGA临床路径和技术标准；同时带动基层医院，建设老年医学、全科医学人才培养基地。  

Objectives of Study：

1. Popularize intelligent CGA and rehabilitation intervention technologies to the grassroots level, provide more elderly people with early assessment and guidance on healthy lifestyle, and advance the application of CGA, so as to achieve the goal of prevention over treatment; 2. Optimize the CGA clinical pathway and technical standards for different elderly populations in inpatient, outpatient and community Settings; At the same time to drive the grass-roots hospitals, the construction of geriatrics, general medical personnel training base.

纳入标准：

Inclusion criteria

排除标准：

1.急性疾病；
2.慢性病恶化需要紧急治疗；
3.有显著的视力、听力、认知功能下降无法签署知情同意和配合资料收集及后续健康指导、干预者。

Exclusion criteria：

1. Patients with acute diseases;
2. Patients whose chronic diseases worsen and need emergency treatment;
3. The subjects with significant decline of vision, hearing and cognitive function can not sign informed consent and cooperate with data collection and follow-up health guidance and intervention.
