研究目的：

通过社区随机的方式，将社区分为常规管理模式、远程信息/结构化多重危险因素管理模式及信息化指导的康复运动社区，比较在标准化的糖尿病管理的基础上应用互联网络和手机网络管理，个性化的康复运动监测及指导，与按常规管理模式在以下方面的作用： 1） 代谢控制-糖化血红蛋白，低密度脂蛋白-血压2组的绝对差别和达标率的差别； 2） 运动能力； 3） 戒烟； 4） 阿司匹林使用； 5） 心血管并发症中间指标的变化； 6） 糖尿病微血管和大血管并发症； 7） 社会经济学影响；心理学问卷，认知问卷 8） 依从性。  

Objectives of Study：

By communities based randomization, patients with type 2 will be allocated into 3 groups, conventional therapy, tele-communication and computer-assisting standard diabetes care focused on multiple CVD risk factors management, ICF evalution-based individualized exercises monitored by a tele communication instrument;the following outcomes will be compared among 3 groups: 1. metabolic parameters: the level of HbA1c, LDL-c, blood pressure and percentage of patients reached targets set by Chinese diabetes society; 2. exercise capacity; 3. percentage of patients stopping smoking; 4. perecentage of patents used aspirin; 5. itermeidate parameters of atherosclerosis; 6. microvascular and macrovascular complications of diabetes; 7. socio-economic impact; 8. adherence to treatment.

纳入标准：

Inclusion criteria

排除标准：

1）有严重心、肝、肺、肾和其他器官功能不全的患者;
2）晚期肿瘤患者;
3）居住流动性大的患者;
4）不同意长期随访的患者；
5）研究者判断的任何不稳定的内分泌疾病、心理疾病、精神疾病、认知障碍或自身免疫性疾病。
6）研究者判定存在影响依从性（包括运动处方）的医学情况；
—被随访者有任何研究者判断无法完成研究或给被随访者带来明显风险的情况。
7) 存在一定的肢体残疾、严重骨关节、脊柱疾病不能进行运动的或不能耐受运动的；
8）深静脉血栓。

Exclusion criteria：

Patients with
1. Severe CVD,liver,kidney and other diseases;
2. end-stage cancers
3. migrant people;
4. not sign written informed consent;
5. any unstable diseases judged by investigators;
6. Medical conditions affect adhere to treatment;
7. diseases with limbs, skeletal, joints and muslces affect exercises;
8. Deep vein thrombosis.
