研究目的：

[主要目的] （1）针对重度抑郁发作被试，建立rTMS增效MECT治疗方案，明确应用rTMS作用于左侧前额叶背外侧皮质区（DLPFC）对于重度抑郁发作被试的增效作用及安全性。 [次要目的] （1）在治疗前后不同时间节点进行临床评估，明确rTMS联合MECT治疗对重度抑郁发作被试症状改善的特征，明确其短期、长期疗效及持续时间。 （2）探讨rTMS治疗疗效与重度抑郁发作相关神经影像学标志物变化的相关性，揭示其可能的神经机制。  

Objectives of Study：

[Main purpose] (1) For the subjects with severe depressive episodes, establish the rTMS synergistic MECT treatment plan, and clarify the synergistic effect and safety of the application of rTMS on the left dorsolateral prefrontal cortex (DLPFC) on subjects with severe depressive episodes. [Secondary purpose] (1) Carry out clinical evaluation at different time points before and after treatment, clarify the characteristics of the improvement of symptoms of subjects with severe depressive episodes by rTMS combined with MECT treatment, and clarify its short-term and long-term efficacy and duration. (2) Explore the correlation between the therapeutic effect of rTMS and the changes in neuroimaging markers related to major depressive episodes, and reveal its possible neural mechanisms.

纳入标准：

Inclusion criteria

排除标准：

1)存在rTMS、MECT或MRI禁忌症：
2)既往曾有酒药依赖及急性中毒史的患者；有违禁药物滥用历史；有精神病性障碍、双相障碍、强迫障碍患者；
3)颅内有金属异物、带心脏起搏器者、有人工耳蜗植入物者、颅内高压；
4)精神发育迟滞及神经系统疾病、重大躯体疾病；
5)脑外伤史，MRI检查显示脑结构异常；
6)酒精或药物滥用及依赖史。

Exclusion criteria：

1) There are contraindications to rTMS, MECT or MRI:
2) Patients with a history of alcohol and drug dependence and acute poisoning; a history of illegal drug abuse; patients with psychotic disorders, bipolar disorder, and obsessive-compulsive disorder;
3) There are metal foreign bodies in the skull, those with pacemakers, those with cochlear implants, and intracranial hypertension;
4) Mental retardation and nervous system diseases, major physical diseases;
5) History of brain trauma, MRI examination showed abnormal brain structure;
6) History of alcohol or drug abuse and dependence.
