析因分组（即根据危险因素或暴露因素分组）

研究目的：

（1）卒中后认知障碍发生率高，后果严重。进行卒中后认知障碍综合管理，可以进一步充实脑卒中专病的全程管理。同时建立“医-社-研”体系，可以进一扩充本院专病研究库。 （2）卒中后认知障碍的产生与地区生活习惯、文化水平、城市发达情况等密切相关。而深圳以高新科技产业为主，人口组成多样、流动人口较多。依托深圳脑卒中筛查与防治基地进行疾病流行病学调查，可以为本土卒中后认知障碍可以为本土人口数据库提供真实数据。 （3）利用简易MRI评估可帮助将临床患者分型进行分层管理，同时可以利用COX回归方法分析筛选并构建预测模型。  

Objectives of Study：

(1) Cognitive impairment and memory loss are common after a stroke. Approximately 30% of stroke patients develop dementia within 1 year of stroke onset.Long-term management of post-stroke cognitive impairment can get a comprehensive disease follow-up plan. At the same time, investigator can establish a patient based "hospital- community - research institute" database for further investigation. (2) The occurrence of post-stroke cognitive impairment is closely related to local living habits, cultural level and urban development. Shenzhen is dominated by high-tech industries, with a diverse population. Relying on the stroke screening to carry out disease epidemiological survey can provide real data for the local elder population database. (3) Simple MRI scale can help to classify clinical patients into different levels, and can be used to analyze, screening to build prediction model.

纳入标准：

Inclusion criteria

排除标准：

1）严重的肢体活动障碍，影响生活；
2）既往有明确的痴呆，或者影响认知功能的精神或神经系统疾病，没有使用影响智能的神经、精神相关用药；
3）重大的躯体疾病、颅脑外伤史，严重的就酒精、药物滥用史；
4）进行过卒中后的溶栓、取栓等内外科治疗手段；
5）严重的失语、失认、失读等会影响认知训练中沟通的功能损伤；
6）早期即出现锥体外系症状；早期即有幻觉等行为学异常或者精神症状；
7）由创伤、颅脑手术、血管造影、颅内肿瘤等引发的卒中；
8）其他不能配合完成随访的情况。

Exclusion criteria：

1) Have severe physical activity disorder after stroke;
2) Had definite dementia or mental or nervous system diseases that affected cognitive function; did not use nerve and spirit related drugs that affected intelligence in the past;
3) Had brain injury history, serious alcohol, drug abuse history;
4) thrombolysis or thrombectomy after stroke, or recieved other internal and external treatment;
5) Serious aphasia, loss of recognition, loss of reading which will affect the communication.
6) Had extrapyramidal symptoms in early stage, or had hallucinations and other behavioral abnormalities or mental symptoms appeared in the early stage;
7) Stroke caused by trauma, craniocerebral surgery, angiography, intracranial tumor, etc.;
8) Other cases interrupt the follow-up.
