研究目的：

1.调查0-3岁神经功能障碍儿童的营养状况。 2.研究营养状况对功能状况的影响。 3.制定神经功能障碍儿童标准化营养干预。  

Objectives of Study：

1.To investigate the nutritional status of children with neurological dysfunction aged 0-3 years. 2.To explore the impact of nutritional status on functional status. 3.Develop standardized nutritional interventions for children with neurological dysfunction.

药物成份或治疗方案详述：

1.给与家长进食相关指导：评估营养不良程度、分析营养不良原因，加强家长的营养意识，给患者提供个体化的饮食指导 2.如有以下情况，可选择鼻胃管插管/胃肠造瘘： 1）进食时间明显延长（>4-6h/d），限制儿童的社交活动； 2）经口喂养家长和孩子均存在压力； 3）喂养过程中的误吸导致反复呼吸道感染或影响进食的乐趣； 4）经口喂养营养干预，仍存在持续的体重增长不良。 5）存在以下指征与消化科沟通造瘘手术：>3个月的肠内营养、存在严重吞咽咀嚼障碍、严重胃食管返流的患者建议胃或肠造瘘。 3.能量干预：给予特殊医学用途配方乳：针对患儿营养不良的原因选用不同种特殊配方奶粉干预，实现体重的追赶式生长。存在吞咽功能障碍、胃食管反流、正常热量食物容量不耐受患儿采用乳清蛋白配方高热量（100kcal/100ml）配方乳,以提高热量，减少胃容量并降低反流危险。 

Description for medicine or protocol of treatment in detail：

1.Provide parents with guidance on eating: assess the degree of malnutrition, analyze the causes of malnutrition, strengthen parents' awareness of nutrition, and provide patients with personalized dietary guidance 2.Nasogastric tube intubation/gastroenterostomy can be selected if: 1) Significantly prolonged feeding time (>4-6h/d), limiting children's social activities; 2) Pressure exists in both parents and children during oral feeding; 3) Aspiration during feeding can lead to repeated respiratory infections or affect the pleasure of eating; 4) There is still persistent poor weight gain after oral feeding nutrition intervention. 5) Communicate with gastroenterology department for fistula operation with the following indications: patients with enteral nutrition > 3 months, severe swallowing and chewing disorders, and severe gastroesophageal reflux are recommended to have gastric or enterostomy. 3.Energy intervention: Formula milk for special medical use: Different kinds of special formula milk powder were selected for the intervention to realize the catch-up growth of weight according to the causes of malnutrition in children.Children with dysphagia, gastroesophageal reflux and intolerance to normal caloric food volume adopt whey protein formula with high calorie (100kcal/100ml) formula to increase calories, reduce gastric volume and reduce the risk of reflux.

1.存在严重呼吸、消化系统疾病；
2.急慢性肝病、肾病、结核等消耗性疾病；
3.先天性心脏病，血液系统、遗传代谢性疾病。

Exclusion criteria：

1.Serious respiratory and digestive diseases;
2.Acute or chronic liver diseases, kidney diseases, tuberculosis and other consumptive diseases;
3.Congenital heart disease, hematological system diseases, inherited metabolic diseases.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

研究者根据受试者的原始及观察记录，将数据及时、完整、正确、清晰地载入病例报告表，录入时采用相应的数据库系统双人双机录入，之后对数据库进行两次比对。电子数据文件分类保存，并有多个备份保存于不同磁盘或记录介质上，妥善保存，防止损坏。

According to the original and observation records of the subjects, the researcher loaded the data into the case report form in a timely, complete, correct and clear manner.
