研究目的：

痔病是一种常见病、多发病，成人发病率高达49.14%，以便血及痔核脱出为主要临床表现。手术是治疗Ⅲ度混合痔的主要方式。Longo于1998年提出的PPH治疗痔脱垂成为传统痔切除术的替代方法。一些随机对照试验显示，与传统痔疮切除术（conventional haemorrhoidectomy，CH）相比，吻合器痔疮切除术（Stapled haemorrhoidopexy）具有术后疼痛少、工作活动恢复快的优点，但有一个明显更高的复发性脱垂率。为了克服这个缺点，对于重度脱垂性痔病，为了提高临床疗效，降低并，保护肛门功能，我们科室近几年提出了大视窗吻合器痔上粘膜切除吻合手术结合外痔弧形切除缝合保留肛管上皮手术，从理论上讲该项技术较好地处理了内痔和痔上粘膜脱垂，降低内痔再脱垂发生率，保留肛垫和齿线，保护肛门感觉和控便功能；强调完全保留肛管上皮，将原来肛管内痔核放射状切除改为肛缘外外痔弧形切除缝合，外痔切除彻底，且不会引起狭窄及肛管创面愈合迟缓。该项技术较好的兼顾了手术的彻底性和功能保护的问题。前期临床研究表明：该技术具有适应症广，临床疗效好，痛苦小，恢复时间快等优点。 本课题组设计了大视窗吻合器结合保留肛管上皮术，前期在国内开展了大视窗吻合器结合保留肛管上皮术治疗重度脱垂性痔病的探索性研究，发现所研究的新方法能较好的解决上述问题。并本研究的目的是探索保护肛门功能并提高重度脱垂行痔疮的治愈率的新途径。  

Objectives of Study：

Hemorrhoids is a common disease and frequently occurring disease. The incidence rate of adults is 49.14%. The main clinical manifestations are haemorrhoidectomy and haemorrhoidectomy. Operation is the main treatment for grade III mixed hemorrhoids. Longo proposed PPH in 1998 as an alternative to traditional hemorrhoidectomy. Some randomized controlled trials have shown that stapled hemorrhoidectomy has the advantages of less postoperative pain and faster recovery of work activities compared with conventional hemorrhoidectomy (CH), but it has a significantly higher rate of recurrent prolapse. In order to overcome this shortcoming, for severe prolapse hemorrhoids, in order to improve the clinical efficacy, reduce and protect the anal function, our department in recent years proposed large window stapler hemorrhoids mucosal resection and anastomosis combined with external hemorrhoids arc resection and suture to preserve the anal canal epithelium operation. Theoretically speaking, this technology can better deal with internal hemorrhoids and hemorrhoid mucosa prolapse, and reduce internal hemorrhoids prolapse The rate of occurrence is that the anal cushion and tooth line are retained to protect the anal sensation and control function; the anal canal epithelium is completely preserved, and the original radial resection of hemorrhoids in the anal canal is replaced by arc resection and suture of external hemorrhoids outside the anal margin. The external hemorrhoids are completely removed without causing stenosis and delayed healing of anal canal wound. This technique takes into account the problems of the thoroughness of the operation and the protection of the function. Preliminary clinical studies show that this technique has the advantages of wide indications, good clinical efficacy, less pain and fast recovery time. The research group designed a large window stapler combined with anal canal epithelial preservation surgery. In the early stage, we carried out exploratory research on the treatment of severe prolapsed hemorrhoids with large window stapler combined with preservation of anal canal epithelium, and found that the new method can better solve the above problems. The purpose of this study is to explore a new way to protect anal function and improve the cure rate of severe prolapse hemorrhoids.

纳入标准：

Inclusion criteria

排除标准：

①已行过混合痔手术者。或合并其他术式，或伴有其他肛肠疾病的患者.
②妊娠期、哺乳期及月经期的女性。
③合并直肠癌、直肠息肉、结核、克罗恩病等其他直肠肛门疾病者。
④有呼吸、消化、循环、血液系统严重疾病,肝肾功能明显异常者。
⑤急性炎症性或血栓性外痔。

Exclusion criteria：

1. Patients who have undergone mixed hemorrhoids surgery. Or combined with other operations, or with other anorectal diseases;
2. Women in pregnancy, lactation and menstruation;
3. Patients with rectal cancer, rectal polyps, tuberculosis, Crohn's disease and other anorectal diseases;
4. Patients with severe diseases of respiratory, digestive, circulatory and blood systems, and obvious abnormal liver and kidney functions;
5. Acute inflammatory or thrombotic external hemorrhoids.
