研究目的:

本研究的目的是探讨基于医疗物联网远程管理平台的现场与远程相结合的干预模式对GDM患者的干预和随访管理效果,主要评价妊娠期糖尿病患者对饮食、运动、血糖和体重管理的依从性,妊娠中晚期的血糖、血脂代谢状况,以及产科妊娠结局的改善效果。  

Objectives of Study:

The purpose of the present study was to identify what management model may improve the compliance and outcomes with diet, exercise, blood glucose and weight controlment when managing GDM. The particular focus of this study was interpersonal interactions between GDM management and obstetric pregnancy outcomes, doctors and nurses can better educate and support pregnant women to manage GDM.

药物成份或治疗方案详述:

干预:采用现场的医学营养、运动和自我血糖监测指导,并联合运用基于物联网的远程营养、运动、血糖和体重等监测与追踪管理的模式对该组孕妇进行管理。开发基于医疗物联网的APP软件,试验组研究对象应用该软件实现每日膳食种类和数量的录入,运动、指尖微量血糖、体重等监护数据的自动采集或录入等功能。全程采集的所有数据指标可通过云端数据中心传输到医院的系统进行大数据的分析,出现异常血糖和体重增长等数据时,系统将自动提醒孕妇和医护人员,医护人员接收系统异常值提醒或咨询任务请求后,对异常值和相关指标及其变化趋势进行分析和反馈指导。 

Description for medicine or protocol of treatment in detail:

This study was conducted by the obstetricians, nutritionists, specialist nurses and software engineers. The nutritionists and specialist nurses instructed the two groups how to take medical nutrition, exercise and self-monitor blood glucose one-to-one or by group through one-day outpatient. Engineers developed mother-to-child smart medical cell phone APP software. This application was applied for remotely monitoring and managing the maternal and fetal nutrition and the pregnancys blood glucose based on mIoT technology, where the pregnancy personal basic information, identity authentication, disease history, birth history, current pregnancy status and related risk factors were collected. The subjects of the intervention group recorded their daily dietary type and quantity, exercise, fingertip trace blood glucose, and weight in this APP. All data collected in the whole process was transmitted to the hospital system through the cloud data center. When data such as abnormal blood glucose and weight gain occurred, the system automatically reminded pregnant women and health care workers. And the time the medical staffs received the abnormal signals, they immediately analyzed the abnormal ones and found out the related changing trend. Meanwhile, the medical staffs feedbacked to pregnant women and guide them how to do next.

排除标准:孕前糖尿病、慢性高血压、孕前肥胖、控制不良的甲状腺功能亢进或减退、多囊卵巢综合症;前置胎盘、先兆流产等具有运动禁忌症者。

Exclusion criteria:

The study exclusion criteria were as follows: pregnant women were diagnosed with preexisting diabetes, chronic hypertension, pre-pregnancy obesity, poorly-control hyperglycemia or hypoglycemia, polycystic ovarian syndrome (PCOS), placenta previa. And those who clinically have exercise contraindications, such as threatened abortion.

医学营养和运动干预,以及自我血糖、体重监测管理的指导方式和方法同试验组。与试验组干预不同的是对照组不加入基于物联网APP的远程营养与血糖管理平台,而是遵嘱进行自我饮食和运动的管理,记录饮食和运动日记。同时每天自我监测血糖4次(空腹1 次,餐后3 次,选择餐后2h),连续监测5~7天后,评估血糖达到控制自标后改为每周监测任意2~3天,监测次数同上。经医学营养干预和有效运动1周后,空腹和餐后血糖仍达不到血糖控制标准时应复诊。如空腹血浆血糖>6.1mmol/L,餐后1小时血糖>7.8mmol/L时应遵嘱应用胰岛素进行治疗。
