研究目的：	本研究通过对合并GDM高危因素的我国妊娠妇女进行GDM早期筛查、有效的血糖管理及多种胰岛素干预方法的比较，评估母婴围产期并发症发生率及孕妇产后胰岛细胞功能、DM发生等一系列客观指标，旨在明确对上述人群进行GDM早期筛查的必要性、胰岛素强化干预治疗的安全性和有效性。为确立适合我国国情的上述GDM高危人群更优化治疗方案以确保其生殖健康和围产期安全提供可靠的临床研究依据。
Objectives of Study：	The aim of study was to early screening, effective management of blood glucose, comparison of various insulin intervention and assessment of prenatal complication morbidity and postpartum islet cell function and incidence of diabetes in gestational diabetes risk population. Further, the study was to confirm the necessity of early screening, the safety and effectiveness of insulin intensive therapy, to establish optimize treatment regimens for China gestational diabetes risk population and ensure reproductive health and provide reliable clinical studies based on perinatal security.
药物成份或治疗方案详述：
Description for medicine or protocol of treatment in detail：
纳入标准：	孕妇年龄> 30 岁;肥胖[孕前体重指数(BMI) ≥25 kg/ m2] ;孕前患多囊卵巢综合征或月经不规则;糖尿病家族史(尤其一级亲属);早孕期空腹尿糖阳性;异常产科病史(GDM史、新生儿呼吸窘迫综合征、畸形儿、胎死宫内、巨大儿史)及本次妊娠可疑巨大儿、羊水过多者。
Inclusion criteria	Maternal aged >30 years, obese pre-pregnancy body mass index (BMI) >=25 kg / m2], preconception suffering from polycystic ovary syndrome, or irregular menstrual periods, family history of diabetes (especially first-degree relatives), early pregnancy fasting urine positiveabnormal obstetric history (GDM history, neonatal respiratory distress syndrome, stillbirth and teratismus, macrosomia history) and the suspicious macrosomia, polyhydramnios.
排除标准：	糖尿病酮症或酮症酸中毒、糖尿病高渗状态、糖尿病乳酸酸中毒等急性糖尿病并发症或严重慢性糖尿病并发症；合并先兆流产、妊高征、子痫等患者；充血性心力衰竭（NYHA分级Ⅲ~Ⅳ级）；其他严重伴随疾病；谷氨酸脱羧酶（GAD）抗体阳性；高甘油三酯≥ 5.0mmol/L；有胰腺炎病史。
Exclusion criteria：	Diabetic ketosis or ketoacidosis, diabetic hyperosmolar state, diabetic lactic acidosis and other acute diabetes complications or serious chronic complications of diabetes, threatened abortion, pregnancy induced hypertension, eclampsia patients, congestive heart failure (NYHA class III-IV degrade) and other serious comorbidities, glutamic acid decarboxylase (GAD) antibodies positive; high triglycerides >=5.0mmol/L, pancreatitis dieases).
