研究目的：

1） 本研究将基于前期开展的母婴队列进行前瞻性研究，确立剖宫产与儿童注意缺陷多动障碍的关联及关联强度。 2） 控制剖宫产指征的混杂因素，探索无指征剖宫产与儿童注意缺陷多动障碍关联及关联强度。 3） 从疾病一级预防角度出发，降低注意缺陷多动障碍的发病率，为剖宫产对儿童健康长期效应的研究提供新的思路和理论依据。  

Objectives of Study：

1) to establish the Association and Strength Between Cesarean Section and attention deficit hyperactivity disorder (ADHD) based on a prospective study of a cohort of mothers and infants. 2) to control the confounding factors of the indication of Cesarean section, and to explore the correlation and intensity between the indication of Cesarean section and children's attention deficit hyperactivity disorder. 3) to reduce the incidence of attention deficit hyperactivity disorder (ADHD) from the point of view of primary prevention of the disease, so as to provide new ideas and theoretical basis for the study of the long-term effect of Cesarean section on children's health.

纳入标准：

Inclusion criteria

排除标准：

1.孕期并发症（妊娠期糖尿病、高血压、子痫）；
2.孕妇及配偶患有高血压，甲状腺功能疾病，HIV及性传播疾病等感染性疾病等严重疾病。

Exclusion criteria：

1. Exclude pregnant women with pregnancy complications (gestational diabetes, hypertension, ECLAMPSIA);
2. Pregnant women and their spouses suffer from: hypertension, thyroid disease, HIV and sexually transmitted diseases, such as infectious diseases and other serious diseases.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1.任何有关本项研究结果的公开报告不会披露随访儿童任何的个人信息。本研究的病例资料可能被研究者、上海市儿童医院伦理委员会、研究申办者查阅。 2.随访儿童的医疗信息、病历资料将被编码管理，且不包含任何可关联到个人的任何可识别信息（姓名、出生日期、学校、家庭地址、电话、病例资料号、社保卡号等）。 3.研究数据将在研究结束后以书面文字及电子档案形式保存5年，该数据与随访儿童隐私信息的关联编码由研究者所在单位（部门）的安全地方双人双锁保管。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Any public report on the results of this study will not disclose any personal information about the children who were followed. The case data of this study may be consulted by the researcher, the Ethics Committee of Shanghai Children's Hospital and the study sponsor. 2. The medical information and medical records of the follow-up children will be coded and managed, it does not contain any identifiable information (name, date of birth, school, home address, phone number, medical record number, social security card number, etc.) that can be associated with the individual. 3. The study data will be kept in a written and electronic file for 5 years after the conclusion of the study. The association codes of the data and the Children's privacy information will be kept in a safe place with two people and two locks in the researcher's unit (department) .
