研究目的:

开发一种人类甲状腺多基因突变检测试剂盒(高通量测序法)。该试剂盒覆盖DNA突变及RNA融合,降低测序成本的同时,降低检测周期。该试剂盒需满足从穿刺样本提供的微量新鲜组织中同时提取足量的DNA和RNA,用于对于甲状腺肿瘤诊断最重要的点突变和融合基因检测。  

Objectives of Study:

The aims of this study are to develop a diagnostic kit to detecting mutation multiple genes in thyroid gland tumors. This diagnostic kit will detect the mutations in DNA and fusions in RNA. The test will reduce the cost as well as shortening the overall run time. This kit will isolate DNA and RNA simultaneously. It is expected successfully detect mutation from the very small amount of Fine-Needle Aspiration sample harvested from patient with thyroid tumor.

受试者术后的石蜡包埋,福尔马林固定的甲状腺标本的病例诊断为本项目的金标准

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

The final pathologic diagnosis from the subjects’ paraffin embedded, formalin fixed surgical specimens will be the golden standard of this study. Subjects’ surgical paraffin embedded, formalin fixed specimens will be the golden standard of this study.

甲状腺肿瘤样本中22个基因所可能含有的点突变、小片段插入\缺失、融合。

Index test:

The point mutation, indel, fusion which are possibly detected in 22 designed thyroid gland tumor related genes.

目标人群(可以是某种疾病患者或正常人群,详细描述其疾病特征,注意应纳入符合分布特点的全序列病例,具有良好的代表性)

本项目拟检测250名符合条件的受试者。检测年龄大于18岁,性别不限,患有一个及以上的甲状腺结节的受试者,征得同意愿意提供样本参加基因检测。受试者的样本穿刺后制成细胞涂片,经细胞学医生诊断后,结果为甲状腺结节Bethesda分级III、IV、V级者的样本进行高通量基因测序检测。

Target condition (The target condition is a particular disease or disease stage that the index test will be intended to identify. Please specify the characteristics in detail; the population should has a complete spectrum and good representative):

This study plan to recruit 250 subjects. All subjects should be older than 18 years of age. Both male and female are fit for this study. Those patients whose Fine-Needle Aspiration cytologic reports are Bethesda grade III, IV, V will be recruited into this study.

容易混淆的疾病人群(即与目标疾病不易区分的一种或多种不同疾病,应避免采用正常人群对照的病例-对照设计):

患者的甲状腺结节为甲状腺原发的不含甲状腺的肿瘤或瘤样病变,如淋巴瘤、软组织来源肿瘤等,或其它器官的转移性肿瘤。

Population with condition difficult to distinguish from the target condition, the normal population in a case-control study design should be avoid:

The subjects’ thyroid gland nodules are tumor or tumor like lesion which are not originated for thyroid glands, for instance,lymphoma, soft tumor, etc. The metastatic tumors of thyroid glands are also the condition difficult to distinguish from the target thyroid gland nodules.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

临床病理数据和临床病史数据采集于我中心临床及病理数据库。随访数据通过电话随访、门诊随访获得。由两名经验丰富的、不知道研究目的的试验人员收集数据。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Clinicopathological features and clinical history will be obtained from our institutional database and reviewed using a standard data collection form. Follow-up data were obtained by telephone and/or clinic visit. Data collection was performed by trained staff (two people). The trained staff, however, did not know the purpose of the study.
