研究目的:

本研究旨在探索与常规人工任务导向性训练(TOT)相比,应用力反馈机器人强化纠正上肢运动障碍对脑卒中患者上肢运动功能障碍康复的有效性和安全性,以及临床经济学评估。  

Objectives of Study:

The purpose of this study is to explore the efficacy and safety of using force feedback robot to strengthen the correction of upper limb dyskinesia in stroke patients with upper limb motor dysfunction rehabilitation compared with conventional manual task-oriented training (TOT), as well as clinical economic evaluation.

纳入标准:

Inclusion criteria

排除标准:

1) 伴有意识障碍或认知功能障碍 [简易精神状态检查 (Mini- Mental State Examination, MMSE)≤20];
2) 肌张力过高,Ashworth 分级≥Ⅲ级;
3) 患有严重的高血压(收缩压>200mmHg,舒张压>110mmHg)、糖尿病血糖控制不佳或合并肝、肾、心脏等重要脏器功能障碍等重大疾病等;
4) 肩关节伴严重疼痛;
5) 严重肩、肘关节被动关节活动度受限;
6) 难治性或多发性癫痫者;
7) 有严重的上下肢关节疾病等既往史;
8) 内置心脏起搏器或体内植入其他金属医疗器械;
9) 不稳定性骨折者,严重的骨质疏松者;
10) 严重的中风发作前残疾,(modified Rankin Scale, mRS=5);
11) 视力、听力或理解能力存在严重障碍者;
12) 怀孕或哺乳期的妇女;
13) 已知对智能力反馈式机器人会产生变态反应、超敏反应或不耐受;
14) 正在参与另一项临床试验;
15) 研究者认为存在可能损害受试者或者导致受试者无法满足或执行研究要求的任何状况。

Exclusion criteria:

1) Accompanied by disturbance of consciousness or cognitive dysfunction [Mini- Mental State Examination (MMSE) <=20];
2) Muscle tension is too high, Ashworth grade >= III;
3) Suffering from severe hypertension (systolic blood pressure > 200 mmHg, diastolic blood pressure > 110 mmHg), poor control of diabetes or Combined with liver, kidney, heart and other important organ dysfunction and other major diseases;
4) Shoulder with severe pain;
5) Severe shoulder, elbow passive joint range of motion is limited;
6) Refractory or multiple epilepsy;
7) A past history of severe upper and lower limb joint disease;
8) Built-in cardiac pacemakers or other metal medical devices;
9) Unstable fractures, severe osteoporosis;
10) Severe pre-stroke disability,(modified Rankin Scale, mRS = 5);
11) Severe impairment of vision, hearing or comprehension;
12) Pregnant or lactating women;
13) Known allergy, hypersensitivity or intolerance to intelligent force feedback robots;
14) Participating in another clinical trial;
15) Any condition that, in the opinion of the investigator, may impair the subject or cause the subject to be unable to meet or perform the study requirements.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

本研究的数据类型主要通过CRF表进行收集,必须及时、准确地填写CRF;对于任何缺失/异常数据予以解释。本研究中涉及的实验室检查结果将出具具体检查报告,并记录于CRF表中。此外,将建立瑞云公共数据库专门用于保存CRF中的原始数据,所有研究人员仅有一个账户号,且有密码保护;未经研究主要负责人同意,不得将该账户密码分享给与本研究无关的其他人员。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

The data type for this study is primarily collected via CRF forms, which must be completed in a timely and accurate manner; any missing/abnormal data will be explained. Laboratory test results involved in this study will be provided with specific test reports and recorded in CRF. In addition, a public database of Ruiyun will be established specifically for storing the original data in the CRF. All the study personnel have only one account number and have password protection; without the consent of the principal investigator, the password of this account should not be shared with other personnel irrelevant to this study.
