研究目的:

脑卒中后认知障碍的恢复是目前研究的重点及难点。临床实践研究显示认知-运动双重任务训练在促进认知和神经功能恢复方面效果显著。但相关脑功能重塑机制尚不明确。本研究拟采用rTMS及经红外成像技术明确健康受试者大脑运动皮质区与认知皮质区之间的神经通路,进而比较健康受试者和脑卒中患者运动-认知脑功能连接的差异。最后,采用不同rTMS刺激模式干预脑卒中患者,探讨运动-认知脑功能连接在脑卒中后认知功能障碍的重塑机制,为运动-认知双任务训练模式促进脑卒中后认知功能障碍的临床康复提供理论依据。  

Objectives of Study:

The recovery of cognitive impairment after stroke is the focus and difficulty of current research. Clinical practice shows that cognitive-motor dual task training is effective in promoting cognitive and neurological function recovery. However, the mechanism of brain functional remodeling is still unclear. In this study, rTMS and fRNIS are used to identify the neural pathways between motor cortex and cognitive cortex in healthy subjects, to compare the differences in motor - cognitive brain functional connections between healthy subjects and stroke patients. Finally, different rTMS modules are used to intervene in stroke patients to explore the remodeling mechanism of the motor - cognitive brain function connection in cognitive dysfunction after stroke, so as to provide a theoretical basis for the motor - cognitive dual task training mode to promote the clinical rehabilitation of cognitive dysfunction after stroke.

纳入标准:

Inclusion criteria

排除标准:

①额叶损伤;
②体内有金属植入物、心脏起搏器或颅骨缺损等rTMS治疗禁忌症;
③既往有脑肿瘤、脑外伤、癫痫病史及癫痫发作风险;
④脑卒中前存在认知功能减退;
⑤任何神经精神疾病及明显的情感障碍可影响测试结果;
⑥任何影响评估及治疗的其他因素。

Exclusion criteria:

1. Patients with frontal lobe injury;
2. Patients with contraindications to rTMS such as metal implants, pacemakers or skull defects;
3. Patients with previous history of brain tumor, brain trauma, epilepsy and seizure risk;
4. Patients with cognitive impairment before stroke;
5. Patients with any neuropsychiatric diseases and obvious emotional disorders that can affect the test results;
6. Patients with any other factors affecting the assessment and treatment.
