研究目的:

(1)将多模态CT及MRI等关键技术运用于早期识别、定量评估急性缺血性脑卒中疾病,通过定量指标反映急性缺血性脑卒中的最佳组织窗,为选择最佳影像学技术提供理论依据; (2)通过探究多模态CT及MRI等关键技术定量指标与急性缺血性脑卒中患者的临床症状发生、发展及转归、预后的相关性,找到能够定量反映急性脑梗死患者严重程度及预后相关的影像学标志物; (3)通过高分辨磁共振血管壁成像对颈动脉斑块形态学、血流动力学、斑块内成分定量分析和斑块炎症的定性评估,探究动脉粥样硬化患者发生缺血性脑卒中的潜在机制与其可能进展结局之间的联系; (4)利用传统机器学习和深度学习,定量分析急性卒中过程中脑实质代谢、血管通透性以及侧支循环变化,建立一套急性脑卒中疾病治疗方案选择及后期功能预后的预测模型,用于急性脑卒中患者的治疗指导及治疗情况预测; (5)建立用于急性缺血性脑卒中的真实世界临床队列研究,确立脑卒中后影像学相关扫描参数的阈值及范围,并优化关键技术指标及图像分析技术,确立用于为临床治疗提供客观依据及疗效评价的影像学技术分析系统,通过反映侧枝循环脑血管变化及组织微观结构与微循环变化,为临床转归与预后预测提供理论支撑。  

Objectives of Study:

(1) Multimodal CT, MRI and other key technologies were applied to early identification and quantitative evaluation of acute ischemic stroke disease. Quantitative indicators were used to reflect the best tissue window of acute ischemic stroke, providing theoretical basis for selecting the best imaging technology; (2) Objective to explore the correlation between the quantitative indexes of multimodal CT and MRI and the occurrence, development, outcome and prognosis of patients with acute ischemic stroke, and to find the imaging markers that can quantitatively reflect the severity and prognosis of patients with acute cerebral infarction; (3) Objective to explore the relationship between the potential mechanism of ischemic stroke in patients with atherosclerosis and its possible progression outcomes by high-resolution magnetic resonance angiography (hrmri) for carotid plaque morphology, hemodynamics, quantitative analysis of plaque composition and plaque inflammation; (4) Traditional machine learning and deep learning were used to quantitatively analyze the changes of brain parenchyma metabolism, vascular permeability and collateral circulation in the process of acute stroke, and a set of prediction models for the selection of treatment options and late functional prognosis of acute stroke were established, which could be used for the treatment guidance and prognosis prediction of acute stroke patients; (5) Objective to establish a real-world clinical cohort study for acute ischemic stroke, establish the threshold and range of imaging related scanning parameters after stroke, optimize the key technical indicators and image analysis technology, and establish an imaging technology analysis system to provide objective basis for clinical treatment and curative effect evaluation And microcirculation changes provide theoretical support for clinical outcome and prognosis prediction.

Inclusion criteria

排除标准:

1.不能完成磁共振检查,体内有金属植入物及幽闭恐惧症;
2.图像质量不满意,不能完成定量分析;
3.大面积脑梗死、脑出血、脑肿瘤、炎性脱髓鞘疾病及其他神经系统疾病患者。

Exclusion criteria:

1. Unable to complete MRI examination, metal implants and claustrophobia in the body;
2. The image quality is not satisfied and quantitative analysis cannot be completed;
3. Patients with massive cerebral infarction, cerebral hemorrhage, brain tumor, inflammatory demyelinating disease and other nervous system diseases.
