研究目的：

本研究旨在探究基于术中脑组织氧饱和度的麻醉管理与POD发生率之间的关系。我们以术中脑组织氧饱和度≤50%或者下降超过基线值的20%为暴露因素，从而研究POD的发生率是否与术中脑组织氧饱和度有关。  

Objectives of Study：

The objective of this study is to detect the influence of rSO2 on POD in the elder patients undergoing hip arthroplasty. In the meantime, the severity of POD, the sleep quality after operations, length of hospital stay and mortality will be noticed.

药物成份或治疗方案详述：

检查患者头的位置以及各管路是否通畅，保证患者的大脑有足够的氧供，维持MAP＞60mmHg；当SpO2异常时，增加SpO2；如果PaCO2＜40mmHg，则将PaCO2调整至40mmHg以上；增加麻醉深度或者降低病人体温的方法来减少脑组织的氧耗 

Description for medicine or protocol of treatment in detail：

Reposition the head to ensure the blood supply of brain, Maintain MAP＞60mmHg; If SpO2 is unnormal, keep it in a normal rage; If PaCO2＜40mmHg, increase PaCO2. Increase the depth of anesthesia or lower the patient's temperature. 

纳入标准：

Inclusion criteria

排除标准：

1）术前合并神经系统疾病，认知功能障碍、意识障碍、严重的视力听力障碍；
2）术前合并严重心脑血管疾病，心功能NYHA分级Ⅲ级及以上、房颤、心绞痛频繁发作、心脏瓣膜疾病、症状性心律失常等；
3）严重的肺部疾病，如需要长期家庭氧疗、中度以上通气功能或弥散功能障碍；
4）严重的肝肾功能障碍；
5）急症手术、酗酒患者除外。

Exclusion criteria：

1. Patients with nervous system diseases, cognitive dysfunction, consciousness disorder and severe visual and hearing impairment before operation;
2. Patients with severe cardiovascular and cerebrovascular diseases before operation, NYHA grade III and above, atrial fibrillation, frequent angina pectoris, heart valve disease, symptomatic arrhythmia, etc;
3. Patients with severe pulmonary diseases, such as long-term family oxygen therapy, moderate or above ventilation function or diffusion dysfunction;
4. Patients with severe liver and kidney dysfunction;
5. Patients with emergency surgery or alcoholism.
