以概念化理论为依据，从家庭照护服务的提供者角度出发，以满足社区失能老人所需的最低服务需求为基础，构建社区失能老人非正式照护者分级护理能力评估指标体系，为社区失能老人非正式照护者能力评估提供工具，并为发展社区失能老人分级照护事业提供理论参考。  

Objectives of Study：

Based on the conceptual theory, from the perspective of family care service providers, and on the basis of meeting the minimum service needs of the community disabled elderly, this paper constructs a hierarchical nursing ability evaluation index system for the community disabled elderly informal caregivers, which provides a tool for the ability assessment of community disabled elderly informal caregivers, and provides a basis for the development of community disabled elderly hierarchical care on reference.

①　文献研究法 查阅近几年国内外社区失能老人照护需求的文献资料，形成对社区失能老人分级护理需求的科学认识。 ②　比较研究法 针对我国社区失能老人机构照护中的不同实践经验，梳理社区失能老人分级照护的不同需求，总结出社区失能老人分级需求的实践现状，取长补短，构建完善的社区失能老人非正式照护者护理能力评估体系。 ③　调查研究法 调查郑州市各地社区失能老人分级护理现状，通过实地个案访谈、问卷调查，通过数据量化分析社区失能老人分级照护中存在的短板，为社区失能老人非正式照护者分级照护提出客观性的护理能力评估标准，以促改进。 

Description for medicine or protocol of treatment in detail：

1. Literature research method In recent years, the domestic and foreign literatures on the needs of community disabled elderly care were consulted to form a scientific understanding of the hierarchical nursing needs of the disabled elderly in the community. 2. Comparative study According to the different practical experience in the institutional care of the disabled elderly in our country, this paper combs the different needs of the hierarchical care of the disabled elderly in the community, summarizes the practical status of the hierarchical needs of the disabled elderly in the community, and learns from each other to build a perfect evaluation system of the nursing capacity of the informal caregivers of the disabled elderly in the community. 3. Investigation and research method Objective to investigate the status quo of grading care for disabled elderly in communities of Zhengzhou city. Through field case interviews, questionnaire survey, and data quantitative analysis, the shortcomings of hierarchical care for disabled elderly in communities were analyzed. Objective evaluation criteria of nursing capacity were proposed for informal caregivers of disabled elderly in communities, so as to promote improvement.

Inclusion criteria

排除标准：

（1）长期由照顾机构进行照护的老人；
（2）有精神系统疾病或精神病史者；
（3）无法交流、有严重智力障碍者（除阿尔兹海默症）；
（4）拒绝参与本研究者。

Exclusion criteria：

1. The elderly who have been cared for by care institutions for a long time;
2. Patients with mental system diseases or mental history;
3. Patients who are unable to communicate and have severe mental retardation (except Alzheimer's disease);
4. Patients who refused to participate in this study.

非随机对照试验。研究人员采用单纯随机抽样法，从郑州市高新区8个社区中抽取4个社区，然后对社区的老人运用ADL量表进行筛查，选出符合要求的失能老人。

Non-randomized controlled trial. The researchers use simple random sampling method to select 4 communities from 8 communities in Zhengzhou high tech Zone, and then used ADL scale to screen the elderly in the community to select the disabled elderly who met the requirements.
