研究目的：

大范围普及基于“ 云享智能柜” 的远程血压管理模式。为基层医院提供一个智能、 简单、 高效的管理居民慢性病的创新工具，促进我国慢性病管理水平的提升； 采用智能终端设备， 实现数据自动采集、 传输， 确保患者血压数据真实、 及时、 连续， 为学术研究和慢性病的防治提供数据参考和依据， 促进我国慢病防治科研成果转化。  

Objectives of Study：

The remote blood pressure management mode based on the "cloud sharing smart cabinet" is widely popularized. Provide primary hospitals with an intelligent, simple, and efficient innovative tool for chronic disease management of residents, and promote the improvement of chronic disease management in my country; use smart terminal equipment to realize automatic data collection and transmission to ensure that patient blood pressure data is true, timely and continuous. Research and the prevention and treatment of chronic diseases provide data reference and basis to promote the transformation of scientific research results in the prevention and treatment of chronic diseases in my country.

纳入标准：

Inclusion criteria

排除标准：

本研究开始前4周内曾参加过其他临床试验
入组时诊室收缩压≥180 mmHg，和/或舒张压≥110mmHg
尿毒症、肌酐>250umol/l；近三个月有急性心脏病史者
合并恶性肿瘤等终末疾病者
严重认知障碍（听力、视力严重下降）、精神病史、焦虑症或抑郁症，生活不能自理者
重度糖尿病患者（糖化血红蛋白>11%）
有较严重的心脏瓣膜病、心肌病，心功能不良者（病情不稳定，可能经常住院）
房颤、房室传导阻滞等可能影响电子血压计工作的心律失常者及曾发生危及生命的心律失常者
臂围>35cm或者<26cm，无法正常使用现有袖带者
配偶或亲属已纳入本研究者
孕妇或哺乳期妇女
居住不固定者，或经常长期外出者

Exclusion criteria：

1. Participated in other clinical trials within 4 weeks before the start of this study;
2. At the time of enrollment, the systolic blood pressure in the office >=180 mmHg, and/or the diastolic blood pressure >=110 mmHg;
3. Uremia, creatinine>250umol/l; those with a history of acute heart disease in the past three months;
4. Patients with terminal diseases such as malignant tumors;
5. Severe cognitive impairment (severe loss of hearing and vision), history of mental illness, anxiety or depression, and unable to take care of themselves;
6. Severe diabetes patients (glycated hemoglobin>11%);
7. Patients with severe valvular heart disease, cardiomyopathy, and poor heart function (unstable condition, may be hospitalized frequently);
8. Patients with arrhythmias such as atrial fibrillation and atrioventricular block that may affect the work of the electronic sphygmomanometer and those who have experienced life-threatening arrhythmias;
9. Those with arm circumference >35cm or <26cm, unable to use the existing cuff normally;
10. Spouse or relatives have been included in this research;
11. Pregnant or breastfeeding women;
12. People who live in an unfixed state, or who often go out for a long time.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

入组建档可先将患者信息填表，然后根据表格录入“医家医”高血压及相关疾病远程管理平台系统，也可以直接录入系统。 随访可通过病例记录表(Case Record Form, CRF)进行记录
