研究目的：

1. 调研慢性心衰患者的营养与膳食摄入现状，寻求最佳证据支持，并基于循证基础构建个性化心衰膳食管理方案； 2. 应用个性化膳食管理方案进行心衰营养实证研究，为优化心衰患者营养管理、改善患者临床愈后提供依据； 3. 丰富慢性心衰患者的链式营养管理内容，为实施科学而规范的心衰营养管理提供思路。  

Objectives of Study：

1. Investigate the status of nutrition and dietary intake of patients with chronic heart failure, seek the best evidence support, and build personalized dietary management program based on evidence-based. 2. The individualized dietary management program was used to conduct the empirical study on the nutrition of heart failure patients, so as to provide the basis for optimizing the nutritional management of patients with heart failure and improving the clinical recovery of patients. 3. Enrich the content of chain nutrition management of patients with chronic heart failure, and provide ideas for the implementation of scientific and standardized nutrition management of heart failure.

纳入标准：

Inclusion criteria

排除标准：

1. 严重肝肾功能不全；
2. 有精神病史或老年性痴呆；
3. 肿瘤患者。

Exclusion criteria：

1. Patients with severe hepatic and renal insufficiency;
2. Patients with mental history or Alzheimer's disease;
3. Cancer patients.
