研究目的:

观察重复经颅磁刺激联合经颅直流电对亚急性脑卒中患者运动功能和大脑皮质兴奋性的影响。  

Objectives of Study:

To explore effects of repetitive transcranial magnetic stimulation (rTMS) combined with transcranial direct current stimulation (tDCS) on motor function and cortex excitability in subacute stroke patients.

药物成份或治疗方案详述:

选取我院2021年01月到2022年01月在我院神经内科住院和门诊的亚急性期(病程<30天内)脑卒中患者,根据临床实践中rTMS和tDCS具体联合的方法不同,将入组患者分为以下4组: (1)对照(Control)组:仅予以脑卒中神经内科常规药物治疗; (2)低频rTMS (rTMS -)组:在常规药物治疗的基础上仅予以1Hz的低频rTMS治疗; (3)阴极tDCS联合rTMS-(tDCS-/rTMS-)组:在常规药物治疗基础上,依次进行tDCS阴极刺激和低频rTMS刺激治疗; (4)阳极tDCS联合rTMS-(tDCS+/rTMS-)组:在常规药物治疗基础上,依次进行tDCS阳极刺激和低频rTMS刺激治疗。 

Description for medicine or protocol of treatment in detail:

All participants were given routine medicine treatments. The patients of sham group only get routine medicine treatments. 1Hz rTMS stimulation were used for rTMS- group. Meanwhile, the participants of tDCS-/rTMS- and tDCS+/rTMS- groups were respectively applied cathodal and anodal tDCS, followed by 1Hz rTMS over the contralesional primary motor cortex on the basis of medicine treatments, which continued for 4 weeks. 

纳入标准:

Inclusion criteria

排除标准:

(1)其他神经系统疾病导致肢体功能障碍的患者;
(2)存在癫痫和金属植入物等rTMS治疗的禁忌症的患者;
(3)存在严重的认知障碍和精神障碍及存在意识障碍等不能配合等情况。

Exclusion criteria:

1. Patients with limb dysfunction caused by other nervous system diseases;
2. Patients with contraindications to rTMS therapy such as epilepsy and metal implants; and
3. Severe cognitive and mental disorders and inability to cooperate. All the patients enrolled in this study signed informed consent forms and related documents.
