Objectives of Study：

To build an accurate and graded care plan for elderly chronically ill patients with heart failure capacity management as an example and verify its effectiveness.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

① 急性左心衰；
② 合并尿毒症、肝硬化、呼吸衰竭、恶性肿瘤、外伤等；
③ 预期在6个月内接受心脏移植或器官移植术后、严重脑卒中后遗症；
④ 此次入院有休克史、抢救史；
⑤ 目前正接受以下治疗：肠内营养、呼吸机、需定期透析、禁食等；
⑥ 诊断精神障碍、认知障碍、丧失活动能力；
⑦ 存在低钠血症。

Exclusion criteria：

1. Patients with acute left heart failure;
2. Patients with uremia, cirrhosis, respiratory failure, malignant tumor, trauma, etc;
3. Patients with severe stroke sequelae expected to receive heart or organ transplantation within 6 months;
4. Patients with history of shock and rescue;
5. Patients who are receiving the following treatments: enteral nutrition, ventilator, regular dialysis, fasting, etc.;
6. Patients with mental disorders, cognitive disorders and loss of activity;
7. Patients with hyponatremia.

由研究生王倩怡根据估算的样本含量，使用SPSS 26.0随机数字生成器（固定值设置为：20200423）生成随机数字，进一步按照1：1比例随机分组，设定A和B两组，其中A为试验组，B为对照组。

Based on the estimated sample size, the graduate student Wang Qianyi uses the SPSS 26.0 random number generator (fixed value setting: 20200423) to generate random numbers, and further randomly grouped according to the 1: 1 ratio, and set A and B two groups.

盲法：

单盲。护理干预者在开始进行容量管理前将相应的随机信封打开，并严格按照随机信封中的分组进行操作，且不参与疗效评价，不告诉受试者实行的是哪种管理措施。当完整的护理管理结束后，护理干预者回避，由不知分组情况及干预情况的研究人员专门进行疗效评价及向受试者收集相关量表的数据。此外，数据录入及统计学分析也由独立人员进行处理，以确保试验数据收集与分析的真实性与客观性。

Blinding：

Single blind. Nursing interventionists open the corresponding random envelopes before starting volume management, and operate strictly according to the groupings in the random envelopes. They do not participate in the evaluation of efficacy and do not tell the subjects which management measures are implemented. When the complete nursing management is over, the nursing intervener avoids it, and the researchers who do not know the grouping situation and the intervention situation conduct special evaluation of the efficacy and collect data from the relevant scales from the subjects. In addition, data entry and statistical analysis are also handled by independent personnel to ensure the authenticity and objectivity of the test data collection and analysis.
