研究目的：

通过将吞咽障碍康复护理循证证据应用于临床情境中，在组织层面探索实施基于循证的康复护理干预的方法并验证实施效果，在实践者层面提高护士对干预的采纳度和实施率并验证实施效果，在患者层面验证改善健康结局的干预效果，为临床确定实施方法应用证据提供依据。  

Objectives of Study：

To crystallize evidence-based practice plan of dysphagia and apply it at the hospital stroke unit for optimizing rehabilitation outcomes of stroke patients. To inform this development of an implementation intervention, we conducted implementation in clinical practice settings aiming to improve nursing management of dysphagia and adopted level of intervention, as well as to provide implementation method for the clinical managers.

纳入标准：

Inclusion criteria

排除标准：

① 短暂性脑缺血发作患者；
② 严重心、肝、肾功能不全、呼吸衰竭及恶性肿瘤患者；
③ 食管、鼻咽等疾病导致的吞咽障碍患者；
④ 有精神障碍及不能积极配合治疗者等。

Exclusion criteria：

1. Patients with transient ischemic attack;
2. Patients with severe heart, liver and renal insufficiency, respiratory failure and malignant tumor;
3. Patients with swallowing disorders caused by diseases such as esophagus and nasopharynx;
4. Patients who have mental disorders and can not actively cooperate with the treatment.
